What Causes Constant Bleeding From A Small Bump At The Anal Opening?

Question: Dear Doctor,

I will be following up my regular doctor in 2 weeks time but if a sinister cause is suspected then I will book an urgent appointment this week.

For 2 months I have been bleeding from my anus, bright red blood on the toilet tissue but none visible in toilet bowl or in/on stool. Blood appears to be coming from a small bump at the anal opening. Only bleed upon wiping, bleeding stops after wiping.

On self exam I can feel a small bump on left side of my anal opening attached to some fibrous feeling tissue which leads into my anus where it then connects to the anal wall on the left and arches to attach to the right side of the anal wall. This all occurs below the anal valve.

Symptoms: discomfort when sitting, tender to the touch, bleeding upon wiping, very infrequent itching. Upon self exam I feel a small bump attached to enlarged feeling fibrous tissues.

No sense of fullness or feeling of pressure. No change in bowel movement frequency, shape/size/colour.

I also have a mild swelling on my right inguinal canal area, cant feel any lymph nodes and assume it is unrelated.

Hi Dear !! Thanks for your query to HCM.
I reviewed facts of your query and updated picture,in context to your health issues.
I also reviewed your worry lately from this condition.

My Opinion-
In My opinion, in the given situation you have 2 separate health issues as follows-

a- One from Chronic Anal lump which bleeds but not profusely and is from infected external piles with proctitis moslty.

The discomfort on sitting is mostly from congested / strangulated external piles, as is seen from your pictures.

Bleeding and itching is from soddened external piles, which seems to be with spasm of anal sphincter.

Accompanied proctitis in rectum needs to be ruled out, before starting any further treatment.

b- Second health issue seems to be from mild swelling in your inguinal region
which is not related to lymph nodes. I would suggest you to consult a Surgeon and get it examined and investigated

USG of inguinal region would fix any inguinal hernia in your case.

Do's Suggested-

Need to Consult with Surgeon for both the health issues as physical examination is a must in this case.

Proctoscopy under local anaesthetic gel/ and physical examination of right inguinal region for any bulging popping in inguinal area.

Antibiotic cover and planned Surgical Excision with anal dilation under General Anaesthesia, would resolve the issues you have.

If inguinal hernia is confirmed,then its repair at earliest would resolve these 2 health issues.

Next appointment-
After 2 weeks to review treatment .

If you update more information/ details upon these suggestions,I would give further suggestions on medicines to correct your health issues.

Thank you for your assistance, it is very much appreciated. I have a final few questions for your consideration regarding my anal bleeding.

1) Do you suspect this is a benign condition that can wait 2 weeks for a routine appointment with my local doctor?

2) Should I request specialist referral when I meet with my local doctor?

3) I have had and currently do have hemorrhoids but the tissue highlighted in the pictures causing discomfort feels different to my other hemorrhoids. It is firm to the touch like contracted muscle or connective tissue, is it possible for hemorrhoids to present like this?

4) Do you have any advice to limit discomfort and bleeding until I can see my local doctor?

I reviewed facts of your updated query and reviewed the picture,in context to your health issues.
I also reviewed your worries lately from this condition.
After study and review,following are My replies to few more specific queries from you-

Q-1) Do you suspect this is a benign condition that can wait 2 weeks for a routine appointment with my local doctor?
Ans-Dear,though its benign,its in acute stage and is hence causing pain/hardened swelling in area shown in pictures,which would need immediate attention to avoid further complications around the anal area.

Q-2) Should I request specialist referral when I meet with my local doctor?
Ans-Dear,this is a special and complicated acute health issue and would need Specialist Surgeon,who would examine physically and would investigate your case,before planning exact type of surgical correction needed in your case.
Hence you need to request your doctor to refer you to Specialist Surgeon.

Ans-3-Dear,it seems that you have got acute cryptitis which is causing acute anal spasm due to pain/ infection in nearby areas of the pictured part.
The part highlighted seems to be hardened from Acuter cryptitis with accompanied infection and swelling of the anal tissue around the pictured area.
The red bump, seems to be progressive opening for the infective discharge inside / or it could be from physical trauma from hard stools/or friction in the area.
From the facts in the picture,when reviewed with your complaints,You seem to be having-" Infected piles with Acute Cryptitis,along with Fissure in ano-which is acutely swollen at one point".
If not treated urgently as suggested in last reply, it could develop in to further complications of obstruction to Urination along with already partial obstruction of the bowel movement.It could progress to anal abscess and more damage to whole bowel system.

4) Do you have any advice to limit discomfort and bleeding until I can see my local doctor?
Ans-4-Dear just follow medicines as suggested in My last reply,but that too after Consults and script from local doctors.
You can start with following medicines-for 2 days time and reach out to specialist care immediately,so as to avoid further complications in time to come.
Anovate cream locally x 3 times a day
Antibiotics-like Tab Ofloxacin-200 mg
with Anti-inflammatory like-Tab Diclofenac 50 mg x 2 times a day.
Bland liquid porridge-diet with plenty of fluids x during next 1 week till you plan for definitive treatment with Specialist Surgeon.
Sitz bath x with diluted XXXXXXX warm dettol solution x 2 times a day
Rest in head low position to relieve oedema and pain and swelling locally.

Next appointment-
After 2 weeks to review treatment .
