Question: Hi my name is XXXX..for a few months now when I have a bm at times it is difficult to feel the muscle opening to release mb or to end bm..however, I had a colonoscopy a year ago w findings of eternal hemmroids no polps found..a lilttle colities found..biopsy came back normal...so, last week my symptoms changed some w with pain after bm or dual pain all day. Usually have morning BMS daily but sometimes skip 2 days together. Don't wanna eat much at all cause I am worried that constipation will be next..I have discomfort on left side before bm and sometimes after bm..anyway last week I had to keep going to the restroom and passing little stool with pain afterwards off and on for a few hours. Now its hurting everyday. I 'm tired and light headed with urge to use restroom but nothing it seems very irritated. I kinda did my own exam rectal and felt bulging area inside hurt when I touch it. Everything feels swallon internal with pressure and off and on flu like symptoms no fever but feel like I was getting one, chills etc..then it passes. No blood in stool just one time after I strained hard which is all the time..really scared to eat!! I'm 42 yes old female..I can't see my gastro until late July.however I do have a appt on Monday with primary Dr. Had bm yesterday not today. Lower pelvic pain around my back should I be worried?? I am very anxious and a worry wart when it comes to my health..can u help me? Please

Hi XXXX,

I have gone through your medical history carefully and understood your concerns.

1. You are saying to have done a colonoscopy 1 year ago and it resulted normal. Therefore, there is nothing to be worried at this extent.

2. From what you are describing, all your symptoms are related to constipation: pain, feeling tired, pressure, internal hemorrhoids.

What you do wrong is to avoid eating. I understand your bm is very painful, but this is a temporary situation. If you do not eat, your bowel would get it harder to have a movement and it will be worst for you.

To my opinion, you should focus more on trying to prevent/ease constipation:
- eat often and small portions
- consume more foods high in fiber content: cereals, fresh fruits and vegetables (kiwi, pear, etc.)
- increase water intake
- try to keep physically active to help your bm
- keep a good anal hygiene
- use hemorrhoids preparations locally after having a bm

If all these steps won't improve your condition, then, I'd advise to:
- start lactulose to soften your stool
- take psyllium supplement
