so i want to ask should i have to go with testosterone?
Testosterone is low normal. Follow up
Detailed Answer:
Hi XXXX
Welcome to HCM. I have gone through the question and understand your concerns.
You have got Vitamin D and B12 deficiencies.
Do you suffer from any other disorders other than the mood symptoms and change in body habitus? How is you sexual hair in arm pits and groin?
Was the testosterone test done in morning before 9 am? How is your libido (sexual interest)
The testosterone test is lower side of normal. It could be a normal response to decreased energy and dietary deficiencies.
Don't worry I suggest you few thing. Have a good diet habits. Eat plenty of fruits and vegetables. Avoid fast foods, sweets and sugar. Have a regular exercise of brisk walking for 20-30 minutes a day. Stay healthy. Then we will repeat a testosterone in morning before 9 am. If it is still low consult an Endocrinologist.
If I were your treating endocrinologist I would check the volume of your testes, genital hair and sperm analysis. If the tests show that your testes is producing less hormone. I would suggest you testosterone injections.
From the lab reports you have send I feel the hormone parameters will be normal on repeat testing if you follow good lifestyle modifications.
Continue the vitamin D and B 12 supplementations.
Hope I have answered your questions. If you have any further queries I will be happy to help you.

Sir i am very depressed and my body and my face looks like a 15 year old boy.
I have been follwing a very healthy diet from past long.
What should i do next sir....?
Should i go with testosterone treatment?
Start treatment only after repeat tests
Detailed Answer:
Hi,
I truly understand your concerns.
I'll explain things to clarify. Your testosterone levels can be low due to 2 things
1. Your testes is affected. In that the testosterone is low and your LH and FSH is very high. Your testes will be small in size.
2. Your pituitary is affected. In that case your LH, FSH will be low and Testo will be low.
In your case Testo is low normal and LH FSH is normal. So we need physical examination, look for any other disorders like hypothyroidism, other illnesses which will slightly disturb your LH-FSH - testosterone axis.
Two treatment options available for low testosterone are
1. Testosterone injections
2. HCG / Gonadotropin injections which will improve the testosterone and sperm count.
If everything is normal a trial of testosterone treatment for few months can be tried after consulting an endocrinologist.
Hope the answer is clear now.

and can you please tell me what should i do now?
Shall i go for the retest? and don't you think that gel will be better than injections?
and what should i tell to endocrinologist,Like you were talking about the sperm test and testes test?and don't you think that an adult of my age should have a testosterone level of 400 to 600?
Do a retest of hormonal parameters
Detailed Answer:
Hi XXXX
Lower side of normal is still normal.
The problem with testosterone gel or injections is that, we are depending on external hormones. If we provide external hormones, our body will decrease the production of this hormone. That means if you take testosterone from outside it may improve your facial hair, muscle strength but your function of testes will decrease.
When testes produce less hormones, sperm production will also decrease. That is the reason why we don't start testosterone unless we are totally convinced.
So do a blood test after consulting an endocrinologist

And will the external testosterone also increase my bone growth if the growth is incomplete and is there any think to change facial structure or facial bone growth?
and also sir what are the possible side eefect it can have in my body in the long run?
Free testosterone, pooled LH and FSH are some of the parameters...
Detailed Answer:
Hi XXXX
I have noted your concerns. Let me explain all your questions.
1. Hormonal parameters include - free testosterone, pooled LH and FSH and thyroid function tests. Your endocrinologist will help you with this.
2. What testosterone do is preventing osteoporosis- it won't make your bone grow after puberty. Facial bone growth also won't change with testosterone therapy.
3. The main concern in starting testosterone now is - it decreases sperm count and may affect fertility. The other risks include increased hemoglobin, sleep disorders and chance of increasing prostate size.
hope I have answered your questions

What should i do now as you already know my problems?
You may need a pituitary MRI scan
Detailed Answer:
Hi XXXX
I have gone through the reports.
Single value of growth hormone (GH) is not of much significance. This is because GH is secreted in pulses. So sometimes GH reports can be low in normal people.
In your case the LH and FSH is also low, you have features of decreased sex hormones and you say that you look like a 15 year old. Though I haven't examined you, I feel like you could be having pituitary hormone deficiencies (LH,FSH,GH). Please consult an endocrinologist. Further evaluation we do in these cases are IGF1, GH stimulation test, and an MRI scan of pituitary.
I think you should proceed with MRI scanning of pituitary which will help to clear your doubts.
