What Causes Diabetes During Pregnancy?

default
Posted on Wed, 6 Apr 2016
Question: Hello,
I am 29 years old and eight months pregnant - 34th week running first pregnancy.
Off late I have developed high sugar (post fasting sugar) - which is 201 mg/dl and fasting sugar is 93 mg/dl. My BP = 130/90. My gyneac has suggested to take second opinon. I have the following question:
Is this level risky or really very risky for the baby and me?
Do i need to take insulin regularly?
If I have to take insulin - will i get used to it post delivery too?
Any other side effects of this?
Looking for your kind opinion.

Hello Mr XXXXXXX Welcome to HCM . Thanks for writing in to is with your health query.I have gone through your query and understand your concerns.Your Fasting sugar levels are normal although post prandial levels areon the higher side. These are not too risky as your fasting levels are normal.I will advise you to undergo post prandial levels after 2 hours of taking regular food again,as there can be one off reading which is on the higher side. Gestational diabetes subsides in more than 70℅ patients spontaneously so you don't need to take insulin after delivery as in case you develop diabetese that can be controlled by oral medications. Please get back to me with your repeat levels of sugar. Regards Dr Manisha Jain. OBGY and Infertility specialist.

Hello Maam,

I took the test today and however the results are different - attached report. I want to know what needs to be done next? Do i have to take any medicine/insulin after this new report? What do the 2 different reports indicate?

Brief Answer:
Repeat test after 1 week of diet control

Detailed Answer:
Hello dear sorry for the delay in response as I was struck in some emergency.
I would like to put you on diet control for a week and then repeat fasting and post prandial samples along with another test viz HbA1c which will help us decide on the insulin dose if at all needed.
Since the post prandial sugar in repeat sample is 139 which is slightly as we take 120 mg/DL as cut off in pregnant females.
Chances are that you will be controlled on diet alone and might not need insulin at all.
But during this one week follow the diet restrictions strictly and keep a note on fetal movements which should be at least 10 in 12 hours time.
Wish you best of health
Regards
Dr Manisha

Hello Doctor...

As advised by u today again i went for same test and reports are :

Fasting Plasma Glucose = 118MG/DL

2 hrs Post Lunch Plasma Glucose = 175 MG /DL

This change in report everyday .. what does it indicates ? Do u still think sugar is high ? it needs insulin treatment or can be managed by diet?

Brief Answer:
Insulin needed

Detailed Answer:
No dear it is very high and it's unlikely that it will be managed by diet alone.
And next time you get sugars tested get HbA1c levels tested also.
Along with insulin you need strict diet control otherwise insulin dose titration will be very difficult and buy your own glucometer which will be very helpful.
All the best.
