Question: My husband just had major rotator cuff shoulder surgery. His pain is being managed with staggered doses of Norco, Tramadol, and Diazepan. He is having real problems with constipation, which we realize is a side effect, but he is also experiencing intense difficulty urinating. He has been drinking plenty of water; but feels there is pressure on his bladder.
We have given him some x-lax and medimucil, and have seen small success. Please advise for constipation and for the urination problem. Thank you. XXXXXXX XXXX

Mostly he has Retention of Urine due to Enlarged Prostate .

Detailed Answer:
Hello
Thanks for your query , based on the facts that you have posted, difficulty in passing urine and constipation that your husband has is mostly due to acute retention of urine due to Enlarged Prostate .

I would suggest you to take him to ER and get catheterized to drain his bladder which would be kept indwelling for few more days till he is investigated thoroughly .

In the mean while, please consult qualified Urologist for clinical and digital rectal examination and get following basic tests done to confirm the diagnosis-

1) Urine routine and culture.
2) Ultrasound scanning of abdomen and pelvis
3) Serum PSA.

It'll be a good idea to start him on antibiotics like Cefotaxime and Nitrofurantoin twice daily , laxative and Tamsulosin once daily (please get the necessary prescription from his doctor).

Further treatment will depend upon result of these tests and final diagnosis.
