Question: Hello
I have been suffering from discomfort in my scrotum and pelvis area for about 4 months now. I did a CT scan, ultrasound , blood and urine tests and everything turned out negative. My doctor said I had prostatitis. I took antibiotics for 6 weeks and the symptoms did not go away. I started assuming that I have chronic non bacterial prostatitis. I'm staying away from alcohol , caffeine and spices for a while now. Pain had reduced but still there. I realized that my wife has been getting a yeast infection every time I ejaculate inside her vagina. could I have a fungal infection in my prostate since my wife had been getting it? could the diagnosis have been wrong from the beginning?

Hello, greetings! Thanks for posting your query in hcm. I have gone through the array of your symptoms and it looks like you have been suffering from chronic prostatitis.

It maybe have been a case of bacterial prostatitis, with the bacteria being resistant to antibiotics. The treatment of choice in such a case is fluoroquinolones or trimethoprim-sulfamethoxazole. In this case, I would like to know the antibiotics used by your doctor and if they were changed on non-responsiveness of the infection. I have seen patients using antibiotics for three to six months. The end point is tough to decide. The antibiotics take a very very long time to penetrate into the prostatic fluid

If your doctor has changed the antibiotics and the symptoms still persist, then your assumption about this being a chronic non bacterial prostatitis, also known as chronic pelvic pain syndrome maybe right. This condition may be treated by warm bath, prostatic massage, drugs belonging to the class alpha-adrenergic blockers that relax the muscles of the prosate, over the counter analgesics like aspirin and ibuprofen.

about the yeast infection, long term antibiotic therapy like in your case leads to eradication of healthy bacteria hence making it easier for the yeast to invade. Thus, the initial diagnosis of prostatitis was correct and now it is superimposed with a yeast infection.

Bacterial prostatitis cannot be transmitted sexually, but yeast infections can be transmitted during sexual intercourse. Thus, both of you may need a course of otal antifungal therapy like fluconazole (Diflucan) and itraconazole (Sporanox). Additionally, your wife maybe requiring a vaginal cream like Clotrimazole. Thank you for your reply. I took septra for 3 weeks without relief. Then I took Cipro for 4 weeks also without relief. The pain isn't severe but it's there. Sometimes the pain would almost not be noticeable for a few days then intensify later. I take the occasional sitz bath which helps a little.

I am asplenic and I want to avoid taking antibiotics for a long period of time because I want antibiotics to be effective if the time comes and I really need them. I don't want my body to build resistance to them as without a spleen I am succeptable to infection.

Hello, greetings! Thanks for the additional information. First of all as you are asplenic, I hope you are vaccinated with necessary vaccines like influenza vaccine, pneumococcal and meningococcal vaccine.

As your doctor has tried the above antibiotics and there has been no improvement, I presume that you are infact suffering from chronic non bacterial prostatitis.

You do not need to continue antibiotics as they maybe of no use.
Continue having the Sitz bath. Avoid alcohol or consuming too much coffee. Limit tobacco use and spicy foods.

You may also ask your treating doctor to prescribe alpha adrenergic blocker drugs like prazosin. Pain killers like ibuprofen may be taken which will provide you symptomatic relief. You can undergo transurethral microwave therapy, which has proven to be highly effective after consultation with your doctor.
