Hi, thanks for asking from HCM.
I can understand your concern. You are having intractable disequilibrium or vertigo.There are three systems in body which can cause vertigo - ear, cerebellum and eyes. I will mention few causes of it which need to be evaluated to treat it
: Ear disease - Labyrinthitis or vestibulitis, unsafe CSOM can cause such symptoms. Evaluation is by evaluation of ear, vestibular function evaluation, Hallpike method, CT of head and temporal bone cuts.
Kindly consult ENT doctor for it.
: Cerebellar disease - Cerebellar injury, atrophy, mass lesion. It can confirmed by checking cerebellar signs like tremors, past pointing etc. You need to consult neurologist for it.
Confirmation is by getting MRI brain.
: Eye disease - Nystagmus or abnormal rotatory eye movements can cause vertigo. It can be simple checked by examining all eye movements.
After confirmation, treatment of every condition is different.
: Ear - It is treated by
antibiotic therapy like Co-amoxyclav therapy
maintaining hygiene,
avoiding specific position which can cause vertigo
anti vertigo medication like betahistine
: Cerebellar/Eye disease - It is treated by removing underlying cause. If permanent damage, it can not be corrected. It can be suppressed by anti vertigo medication.
: You can try medication which can suppress vertigo like
Betahistine
Cinnarizine
Combination of Cinnarizine and Dimenhydrinate
: Eye exercise which have already been prescribed
: Maintain hydration. Low BP or fluid can lead to vertigo
: Avoid alcohol or smoking (if doing)
