Dear Ms,
The most common cause such symptoms in a known Diabetic and Hypertensive is uncontrolled Diabetis and Hypertension. Possibility of intermittent Hypogylcemia is also to be ruled.
I would require additional information about your medical history
1. How long have been Diabetic and Hypertensive?
2. What medication you are presently on?
3. What are your latest investigation reports?
4. Do you have any abdominal, urinary complaints?
It would be helpful if you could ellaborate in detail the history
In the meantime I would suggest you the following
1. Weekly twic measurement of your Blood pressure: preferable after 20 minutes of rest and in lying down position
2. Complete blood count: To rule out anemia as contributing factor
3. Fasting Blood Sugar( After 8 hours of overnight fasting if possible at 6 am) and Post prandial blood sugar: 2 hours after intake of breakfast
4. Gylcosylated Hemoglobin(HbA1C): blood sugar levels give a spot reading. Whereas Glycosylated Hemoglobin gives Gylcemic(sugar) control over preceeding 3 months.
5. Renal Function test: Blood Urea , Serum Creatinine
6. Opthalmology assessment: To rule out eyes as cause for symptoms. It is very important to get Fundus examination of eyes every 6 months to rule out Diabetic/Hypertensive retinopathy
7. ECG and 2 D Echocardiography: For assessment of heart condition.
