What Causes Elevated AST And ALT Levels?
My dr did not explain what ️these meant . I have been suffering now for 7 weeks with severe abdominal pain mainly in the upper right quadrant, extremely bloated but hard to the touch (making me appear several months pregnant) sudden 20 lb. weight gain and overall aching joints accompanied by vomiting , alternating diarrhea and constipation .

Your AST and ALT are enzymes that signify liver function. They are elevated in fact the AST/ALT ratio is observed to be more than 2.0 which is generally known to be in alcoholic liver disease. However we do not know about GGT levels. your other values are do not have units. Please mention them to understand them better.
Your symptoms and the labs seems to be worrisome for me sitting on the other side. With severe abdominal pain, bloating, vomiting and alternative diarrhoea with constipation I suggest you to immediately reach a ER for further discussion with a doctor who can refer you to a Gastroenterologist.

Do you think these labs are something I should be concerned about? As my dr said everything was pretty normal , which I doubted since I know my body well enough to know when something is wrong and even he decided to rerun the labs for 3rd time , but has not let me know the results. Not normal but common in narcotics overuse

The cause for these raised AST and ALT would be due to narcotics. In such cases these are common finding but not normal. Hence you doctor was not concerned.
You can witness yourself that the AST/ALT ratio is more than 2.0 which is not normal. The individual values of these tests were high as well. GGT is Gamma GlutamylTransferase. This is for sure raised in alcoholics so it may not mean to you as you do not consume any.
Let me tell you again we do not treat a person on the basis of lab results but we treat if there are symptoms that concern the person and us in sync with the labs.
So it is good to repeat the labs frequently or as advised by physician while you are on narcotics.
I know it's not what I'm eating as I have been watching what I ate for nearly 3 months now and have increased the amount of excercise I have been doing .

I could think of liver and gall bladder stones only that could cause weight gain, pain and bloating. Non Alcoholic Fatty liver disease called NAFLD is a common entity. Though there are other reasons that can be previewed but would be less common than these.
So an examination clinically along with Ultrasound of liver and gall bladder most likely pin points the reason.
