What Causes Elevated PSA?

Question: My husband's PSA number was 53 when checked this week. It has been going up, sometimes rapidly, for the last 10 years. He has had 4 biopsies and a rectal MRI. Nothing has been found to cause this. Now his doctor wants to do surgery to explore further. What could be causing his number to be going up and is surgery a good idea?

Thanks for your query ,based on the facts that you have posted it appears that your 70 years old husband has raising trend of PSA off and on for over last 10 years .have

I shall be happy if you can post me few more details to help me in making comments on your problem.
1) Does he have obstructing voiding symptoms like frequency ,poor stream ,urgency nocturia etc suggesting Enlarged prostate which is common at this age .?

2) What is volume of his prostate detected on Ultrasound Scanning and what is the post void urine residue ?

The commonest cause to raise PSA is either cancer of the prostate or acute infection of prostate (Prostatitis).As his repeated prostate biopsys have turned out to be negative for cancer of the prostate ,His raised PSA is mostly due to infection (Prostatitis).

Your Urologist has rightly advised him to get surgery done .Doing an endoscopic resection of prostate will help to
1) Confirm the diagnosis by submitting multiple chips of prostate tissue for biopsy
2) Eliminate the primary source for raised PSA .
3) To make him free of obstructive urinary symptoms source .
