What Causes Elevated SGPT Levels With Altered Lipid Profile?
I had diagnosed with elevated SGPT 96 in October 2013 but Total Bilirubin was normal with 0.4 mg/dl and I was under treatment of Udiliv 300 mg twice a day for two months and SGPT come down to 51 in December 2013 and doctor stopped treatment. But after 3 months in April 2014 sgpt again raised to 106 , now in May 2014 it is 96 units after taking Livomyn syrup for one month but not restarted Udiliv.
Kindly advise for further treatment/medicine. I have diagonsed with two calculus of 1.99 cm & 1.58 cm size in Gall Bladder, in XXXXXXX 2013, but have no pain but only have digestive problems. Not taken any treatment as there is no pain.
I am 55 years, non-diabetic with BP fluctuates between 130/90 maximum and 110/70 after taking Envas 5 Mg. Other medical problems are burning sensation in the sole of both feet and dry skin /skin allergies usually in winter months . Kindly advise

Increased SGPT indicates liver injury.It may be due to many reasons like altered lipid profile,medications etc in your case(though you mentioned only about envas).
It is important to exclude other conditions which can lead to increased SGPT like injury to muscles,heart and kidney problem.
You need few more routine investigations like routine hemogram,random blood sugar(RBS),renal function test(RFT),lipid profile,viral markers,urine RE/ME.Ultrasound of abdomen should also be done.
You should get at least lipid profile and viral markers done and continue tablet Ursodeoxycholic acid 300 mg twice daily for three months more.It will help in regeneration of liver cells.
You should avoid red meat and high fat milk products.
You should avoid fried and junk food and avoid alcohol completely if you take it.
Increase your physical activity in the form of exercise.
You should take lot of fruits and vegetables.
You should continue anti-hypertensives and take limited salt in your diet.
You should get cholecystectomy(surgical removal of gall bladder) done because it is the treatment of choice for gall bladder calculus.It may become cancerous in long term,also it is prone to infections and may lead to cholecystitis,so it is wise to remove gall bladder even if it is asymptomatic.
Gall bladder is also known cause of increased SGPT.
Burning feet may be due to vitamin deficiency like vitamin B6 and B12.You should take vitamin B complex (once daily)empirically.

Thanks for immediate reply. I am submitting results of some more blood reports done earlier for your kind immediate response.
CBC was done in November 2013. Results are within the limits except platelet count was 90,000 below normal range. My platelet count always fluctuate between 90,000 to 1,30,000 in the earlier two tests done in last two years. Lipid profile done in October 2013, cholesterol level was within limit, earlier years also cholesterol was within limit. Sugar is also within limit however Random sugar test done in this May 2014 was 61.6 mg/dl below normal value. KFT which was done one year back was normal. Urine Exam was done in October 2013 was normal. I am going to do RFT & Urine RE/ME & ultrasound of the abdomen as advised by you.
In the meantime kindly advise further on the basis of blood reports mentioned above. I am taking Neurobine fort tablet for burning sensation in the sole of feet, for the last few months, but not much effect for burning sensation and Envas 5 mg for BP. No other medicine I am taking now. Few months back I was treated for gas and light stomach pain & some times loose and some time hard tools. Doctor given me Metrozine tab for 5 days and Dompan tab for 3 weeks. Gas & indigestion problem is continuing now also. I used to take alcohol maximum 2 to 3 pegs usually vodka, two to three days in a month in week ends and parties.
Kindly advise further on the basis of what I mentioned above. I will again contact you after doing further tests & ultrasound.

Low platelet count may be due to autoimmune reasons.You need proper evaluation.
Your SGPT is not very high.Usually three fold increase in SGPT is the cause of concern.
At present you can continue Ursodeoxycholic acid(udiliv)tablet.You should avoid alcohol completely.Alcohol is among one of the reason for increased SGPT(though you are occasional drinker).GGT estimation is important in your case.It is specific for alcoholic liver injury.
Ultrasound of abdomen can give more clues,so you should go for it.
For gas problem you can take tablet pantoprazole 40 mg once daily half hour before breakfast for six weeks.Gall bladder calculus is also associated with acidity and bloating,so you should get cholecystectomy done.
