Question : My Somatostatin is a little high, my TSH is zero. I have pains on right side; I have fatty stools. My pains are worse after a fatty meal. My father died of pancreas/liver problems. My octreotide scan was negative. My bilirubin was slightly high. What could be the problem? Where do I go from here? Could it be that my tumors are too small right now to see?

Hi,

I have gone through your medical history carefully and understood your concerns.

1. When stomatostatin level is increased, in order to determine if there is any neuro-endocrinological tumor, we should evaluate its levels. Can you please provide the exact number for its level?

We can think over a possible tumor only if the actual level of somatostatin is 1000 times higher than the normal range.

2. With regards to TSH, which is zero; I need to know what the levels of T3 and T4 are? We should evaluate the levels of all these 3 hormones first before running into final conclusions.

3. You are reporting to be experiencing:
- pain on right side
- fatty stool especially after eating fatty meal
- slightly high bilirubin

All these symptoms are indicative for possible cholecystitis or pancreatitis. However, I should have a look at the full biochemistry testing you did before running into final conclusions.

4. As long as octreotide scan resulted negative, tumor is automatically excluded. However, still, I need to have the octreotide scan full report for further evaluations.

5. For the moment, I advise to run MRI/ct-scan for further evaluation.

6. You can:
- take pain-killer medications to ease the pain
- avoid fatty meal
- take probiotics
- increase water intake.
