What Causes Elevated Liver Enzymes?

Question: Dear Dr.,

Today (6/11/2014) I received my lab test results. SGPT/ALAT is 60 (was 42 in January). Creatine Phosphokinase is 242 (was 146 in January). Gamma-GT is normal: 37, LDH-L is normal: 235 and Alkaline phosphatase is normal: 68. I am on Ezetrol and Lopid to lower my Triglycerides, which are still high: 242 and reverse a fatty liver. Taking the abovementioned into account, are the elevated values for SGPT/ALAT and Creatine Phosphokinase related to the drugs that I am using or is there another reason?

I am so sorry too hear about this elevated liver enzymes, ALAT and SGPT. From the information you have provided, your medications are most likely at the origin of these elevated liver enzymes. These drugs also cause muscle inflammation that causes and increased in the levels of creatinine phosphokinase(CPK) increase. Please, get in contact with your treating doctor as these are side effects of the drugs you are taking to lower the bad cholesterol. Your doctor would review your numbers entirely and make th decision whether to continue with these medications or not. Besides, do you have any annoying pain in your muscles as increased CPK indicates muscle involvement?

Dear Dr. Ditah,

Thank you for your feedback, which is very much appreciated. I indeed feel slight muscle pain, sometimes combined with a tingling sensation and dizziness. What would you advise to reverse my fatty liver and lower my triglycerides (270)? The combination of Lopid and Ezetrol (1 year already) is not really helping. I am not overweight and
do not have high cholestorol. However, my HDL is low (32) due to lack of sport activities. I do take metformine to prevent high sugar levels, although I am not a diabetic. On the side note, I suffer from gastritis and duodentitis due to bile reflux. For the aforementioned I take Pantecta.

Hi and thanks for following up with me. Do you drink alcohol? If you do, please I strongly suggest that you stop doing so.

Physical exercise is very important. Please, consider regular physical activity on most days of the week. How was the diagnosis of this fatty liver made? Has it been evaluated since diagnosis?

Lowering cholesterol and triglycerides are components of treatment of fatty liver and not exclusive. Physical exercise, avoiding alcohol as well as reduced fats intake are all helpful.

After 1year of treatment with concerning numbers, please contact your doctor for the treatment to be evaluated and a follow up of the state of the fatty liver diagnosis that was made. I do not know what were the elements and test done for this diagnosis. If you have the details, I will be happy to have a look.
