Posted on Mon, 21 Nov 2016
Question: Dear Sir/Madam,

I have problem with higher Uric acid level in my blood. I have consulted my general physician previously and as per his advice I have taken medicine Februic 40 for about 6 months. After that the uric acid level decreases below the lower level when I stopped the medicine as per his advice.
After 4-5 months of stopping the medicine, I have again checked my uric acid level and it becomes 8.7 again which is above the threshold. I am controlling the food intake, avoiding the red meat and alcohol completely and also does not take sweets and cold drinks as well.
What should be done in this case, please advice.

Brief Answer:
Could be kidney not removing Uric acid from the body

Detailed Answer:
Hi XXXXXXX

Uric acid level can increase when the medication is stopped because Gout can occur due to genetic or enzyme deficiency.

So l if you stop the treatment it can come back.

I suggest you get back on the treatment. You may need to consider being in Uric acid excreting drug like Probenecid.

It would be best to discuss this with your Doctor.

You must consume atleast 3 litres of water daily. Do get your kidney function tests done to note your renal function because if kidney is not excreting enough Uric acid then it can increase or build up in your body.

I hope I have answered your query.

I will be available to answer your follow up queries.

Dear Sir,

Thank you for your response. I have already done a test of kidney functions, where the report was OK. Can you please let me know which group of medicine among Probenecid and Febuxostat is better and having less side effect and how long should be continued to cure this problem?

Please note that though I am not taking any medicine till now but my blood sugar level generally remains 110 - 120 at fasting.

Brief Answer:
Need to check your Uric acid excretion in 24 hours to decide

Detailed Answer:
Hi XXXX.

Thanks for the reply.

You need to get back on Febuxostat and also continue following the precautions to avoid Gouty attacks.

If your Uric acid excretion is less than 800mg in 24 hours on general diet, that means you need to take Probenecid to help excrete the Uric acid. For that you need to have normal Creatinine clearance which needs to be tested prior.
