Brief Answer:
It could be because of repeated attacks of pneumonia.

Detailed Answer:
Hello XXXX,

There is a plethora of causes of mediastinal lymph nodes commonest being malignancy, infections and interstitial lung disease like sarcoidosis.

Since you have already got it biopsied twice and its negative for malignancy, the cause for enlarged nodes in your case is most likely infection.

Tuberculosis is one of the major infections which causes mediatinal lymphadenitis but it is seen mostly in people of southeast Asian countries.

You have had bouts of pneumonia which may have led to enlargement of mediastinal nodes. Sometimes due to repeated infections the nodes may remain enlarged permanently and will show on xray or cat scan.

If your treating physician/pulmonologist is sure that it1s not of any grave concern then you need not worry. Take care of your COPD and repeated infections.
