Question: I constantly have the feeling that there is a worm in my inner ear and Eustachian tube, sometimes with pain and sometimes not. I seems to only stop if I am taking antibiotics.

It is possible that you might have Eustachian Tube dysfunction that causes fluid accumulation in the middle ear that can get infected causing ear pain,your ears may feel plugged or full,ringing in your ears etc.This is the reason why the antibiotics are helpful.

There are a number of tests that are used to diagnose a dysfunction of Eustachian Tube and some options of treatment.
I would suggest to consult an ENT doctor for examination.
