Question: My daughters one is of 11 yrs and elder one of 16 yrs, got fever 6 days back .. For almost first 5 days they had 102 and 103 fever , today is 6 th day , both developed rash on palms and soles , with high itchiness.. Till now they were on paracetamol and azithromycin.. I'm surprised how did they get same symptoms like HIGH FEVER and ITCHENESS ..for ITCHING we gave Avil 25mg .. Write now platelets count is 215000, which are in range ..

Thanks for asking on this platform

High grade fever in both the siblings with itchy rashes on palms and soles in both the siblings suggests infection with same organism.In dengue rashes develop in same distribution characteristically following 6th day of high grade fever and this indicates recovery phase.
In most of the cases transition from febrile to afebrile/recovery phase occurs without fall in platelet count or deterioration in general condition.
In present scenario I would like to get NS1 antigen and IgM test done for dengue in both the siblings.
I won't get overly worried as rashes are signs of recovery and platelet count is with in normal range.
Giving antiallergic like Avil,application of calamine lotion with plenty of fluids to maintain dehydration is mainstay of treatment.

Thq sir , now we got NS1 antigen positive , v r giving fluids and kept IV-DNS , NS , normal saline , RL ,dextrose 5D.. My father is a compounder , so he is giving , under doctors supervision ..
When I should repeat the tests again sir ??
When shud I check for platelets and after how many hours ??
And what r the tests name ??
Do I need to worry , I feel so helpless .. Both r scratching like hell, when will it reduce ??

As platelet count is more than 2 lac and there are signs of recovery,it would be better to get repeat platelet count done after 24 hours.
Give plenty of oral fluids to maintain proper hydration.IV fluids usually not needed if oral acceptance is good.No other teats are needed.
Giving avil with application of calamine lotion will help in rashes/itchiness.
Observe closely for any worsening of symptoms like cold periphery,poor oral acceptance,letharginess, decreased urine output or bleeding from any site.
If you notice any of these please consult your pediatrician immediately.

Hello sir

Yesterday we did CBP
11 yrs girls platelets increased to 2.5 lacs , but rash has spread all of the body, specifically arms and legs , reddish skin like appearance .. She is taking solid food and having fluids .. But how many days she will take to reduce this rash .. She is on atarax ..

16 yrs girls platelets are now 1.2 lacs .. This girl is on NS saline , she is not taking food , as u know teenagers .. For itching and rash v r giving Avil injection ..

When will the platelets increase , and what is the count increase generally ..

Hi

Welcome again

Platelet counts usually starts rising after 7-8 days of illness as in your younger child.The rashes may take 5-6 days for complete disappearance.
I won't get worried for younger one as the platelet count is rising and child is also accepting oral feeds well.
But for elder one I would suggest you not to take risk and get her admitted to hospital for monitoring(monitoring of vitals for shock and lab parameters like Hematocrit and platelet count)and proper fluid replacement accordingly.
One should not take risk as fluid management should be done very cautiously particularly in this phase(phase of shock and leakage of capillaries).
