Detailed Answer:
Hi and thank you for asking!
I read your query and understood your concerns.
Your BP and HR aren't well controlled and I would like to suggest adding Ramipril or Diovan to control your BP.
Moreover I would recommend to switch from Diltiazem to Nebivolol. Nebivolol is more effective on controlling BP and HR.
However since these treatment change may have unwanted side effects I would recommend to discuss with your doctor about any changes.
Hope this will be of help!
Wish you health!
