What Causes Fluctuating Blood Glucose Levels?

My mother is suffering from Diabetes (Type 2) since last 10-12 years. Age 58yr.
She is having normal BP and other readings are in normal range except SGPT and SGOT which are 67 and 62 respectively. Also since last few days blood sugar level seems to be fluctuating abnormally i.e. abruptly goes down to 100 and again beyond 200.
Please advise
1) what does it indicates.
2) what might be the reason.
3) which specialist to consult.
4) what precaution need to be taken (short term as well as long term).
5) how to confirm and prevent any further complications.
Nephropathy and Retinopathy are possible complications.
Detailed Answer:
Hello, Sir.
I understand your concern.
Is she currently using any, medication to control her blood sugar and blood pressure.Kindly provide info on this in your follow up.
The liver changes possibly reflect diabetic dyslipidemia.
You need to get her a lipid profile to assess the lipid abnormality on the liver.
The fluctuations in blood sugar might be due to change in medications or cessation of drug therapy. An Hb1ac testing will help to document the overall long term control. This is a mandated test in her case.
The possible complications in diabetic individuals are Nephropathy, Retinopathy and Neuropathy.
This needs screening through Funduscopy and Urine albumin testing. These tests can predict their occurrence quite early.
You need to consult a Diabetologist.
Post your further queries if any.
Thank you.

Lipid profile is normal. HbA1c normally use to be at around 7-8 as per tests done since last two year.
Elevated liver enzymes are drug induced.
Detailed Answer:
Hello, Madam.
I understand your concern.
I apologize for the delayed response.
The elevated liver enzymes in her case are drug related. This drug-GLUCORYL M1 has hepatotoxic potential which msnifests in few individuals.
Impairment of liver function can be either ( with cholestasis and jaundice), as well as hepatitis(elevated SGPT and SGOT), which may progress to liver failure if unchecked.
I advise you to check this with your doctor and substitute with another class of drug.
An ultrasound abdomen is also required in your case,as it will throw light on lver morphology at this point of time.
Hb1ac have to be check on semiannual basis. It should be maintained under 6 for indicating good glucose control.
Post your further queries if any.
Thank you.
