Question: Dear Sir,

I'm a diabetic patient, aged 37 years and I've a diabetes since last 11 years post delivery of my Son. Intiitally it was going well but now a days, its continously fluctuating. I'm using Lantus 14 unit at night 09:00 am because of which my fasting is normal and taking XXXXXXX rapid 4 unit before breakfast and Glynase MF and Daonil during lunch and dinner. Sir after taking novarapid, my sugar becomes low after 2-3 hrs, doctors suggested novarapid during lunch and breakfast and novamix during night but because of this my fasting became 250 and PP became very low 90-100 so I again switched to tablets and currently using novarapid only during breakfast and glynase MF and daonil. Sir, I'm Not getting any proper guidance and I'm continously losing weight and becoming more thinner. I go for a walk, taking proper meals, even not taking any sugar still sufferring a lot.

Pl suggest proper medicine sir as now a days diabetes complication started growing , cramps in my thighs and legs, a confused ming due to fluctuation etc.

Just need a continous guidance and pl provide me your fee details so that I can make online payment also. But pl suggest what should I do, I'm taking thrynorm 50 mg for thyroid also.

Glynase mf&Diaonil should not be taken together

Detailed Answer:
Dear madam i can understand you concern.
i would like to tell you some basic steps in diabetes.
For a better diabetes control,Diabetes management can be divided in to four parts.

Part 1---30% of your diabetes control depends on daily exercise
Part 2---30% on your diet plan
Part 3---30% on your medication
part 4--10% on your stress management.
So in management of type 2 diabetes diet,physical activity and medicines will play equal role.Time maintenance is very important you need to take your food and medication in time.
regarding your medication
Glynase and Daonil both of this medications belongs to same family,so its not advisable to take them both.
Daonil is very old medication and very powerful too.
By using this kind of powerful medications your pancreas will exhaust soon
Which will make your beta cells(cells that produce insulin in pancreas)will go into failure.
in my opinion considering new medications like DPP-4 inhibitors is a good idea.
in market they are known with brand names like januvia and janumet in combination with metformin.
The next medication what you are taking is metformin,which is present in combination with glipizide in Glynase MF.
This drug metfromin is a very good drug and helps in weight reduction.so you should continue it.
In my opinion i would like to suggest you to discuss with your doctor regarding below stated combination
Tab amaryl 2 mg in morning before breakfast.
Tab janumet 50/500 taken twice after food ,both morning and night.
Inj lantus 10 units to betaken once bed time and titrated as per your fasting sugars.
