Brief Answer:
Check oral hygiene

Detailed Answer:
Hi,
Thanks for asking.
I am Dr. Prakash HM and I will be answering your query:

Based on your query, my opinion is as follows:
1. Esomeprazole would not cause bad smell or rat smell.
2. Need to look into dental hygiene, possibly caries or gingivitis.
3. Esomeprazole works at the level of stomach, and would not usually affect breath. If you have excess burping, then need to look into air ingestion during food intake. Avoid slow eating and talking during eating food.
4. If dental hygiene is good, then two weeks of probiotics could help. Also try using mouth wash.
5. For heartburn, reducing acidity is the best method. You can consider changing esmoprazole with pantoprazole or rabeprazole, but proton pump inhibitor therapy is necessary.
No major long term side effects.
If reflux and sour feeling in mouth is also present due to acidity, domperidone treatment along with proton pump inhibitor will be helpful.
Hope it helps.
Any further queries, happy to help again.

Brief Answer:
On good dose, continue. Avoid brandy if possible

Detailed Answer:
Hi,
Thanks for asking again.

1. Sompraz may reduce effect of plavix rarely, however, you are on double dose of plavix than normally prescribed. So, not to worry about both. Ecosprin needs to be continued too.
2. With multiple medication, sompraz is necessary to reduce risk of medication induced gastritis.
3. Cardiac medication compulsory along with both diabetes medication.
4. Also avoid spicy food, stress and take regular meals. You are on good control of sugar levels, continue the same.
5. Brandy not advisable. It can cause gastritis and vary sugar levels.

Hope it helps.

Brief Answer:
Topical treatment better.

Detailed Answer:
Hi,
Thanks for asking again.

You are welcome.

For your last query,
I can suggest you two options:

1. Oral itraconazole

2. Topical terbinafine application on nail like - Tamsil or Solveasy tinea, etc.

You can try topical. If not, you can then go for itraconazole.

I think, you developed gastritis, secondary to terbinafin treatment. Along with esmoprazole, it could reduce stomach upset and improve tolerance. You can take esmoprazole before food regularly until treatment is completed.

Even if you start itraconazole, esmoprazole is advisable.

Hope it helps.
