I am using hemroid creams but each time i use the bathroom it hurts

Hi,
I understand your concerns.
1. Having hemorrhoids is indicating that you are also having problems with constipation as long as colonoscopy was negative. Each time you have a bowel movement with hard and difficult to pass stool, you'll also have an episode of hemorrhoids flare ups.
2. The very first thing is to avoid constipation:
- increase fluids (water) intake
- be more physically active
- go to toilet each time you feel the need; do not delay
- consume more foods high in fiber and water: fruits (prunes, figs, apples, soups, etc.); vegetables, etc.
- avoid meat, dairy and other foods (e.g. banana, etc.) that would aggravate constipation
- avoid anal sex
- if you have several days without passing the motion, please use anal enema to ease emptying of your bowels
- continue using hemorrhoid creams (e.g. H-prep) after having a bowel movement
3. With regards to the lump in your penis, you can send a photo to me for a further evaluation. If your doctor already examined it, and concluded it's due to rough sex, and you have a history of having a rough sex, then, nothing worrisome. It will be gone after a few days. Avoid having sex meanwhile and keep a good local hygiene.

1. If the hemorrhoids are persisting, notable and painful, then, a further evaluation by proctologist (surgeon specialized in anal problems) is advised.
After physical examination, he/she will decide whether you are candidate for surgery or not depending on the size? consistency? appearance? and the ability to get back to normal appearance.
2. With regards to the lump in you penis, as asked earlier I need to have a look at it; so , kindly upload a photo. So far, no photo is in.
If the lump has been there for over a year and you are not having any pain at all; then, genital wart is suspected. I need to have a look at the lump to give a final conclusion.
