What Causes Frequent Urination And Pain In Lower Pelvic Area?

Question: I am 61 yrs old male MD with uneasy feeling ,nodysuria, no burning micturition, no discharge,only increased frequency and dull pain in my lower pelvic area. I tried Levaquin 500 mg po ,2 stat and then one po QD. I felt ok for 2-3 days then the dull pain returned.I started Cipro 500mg po for 2 doses I still have the dull ache and increased frequency? it is hard to get a quick apt with a urologist on a weekend !!
Q I have a past HX of prostatitis that responded to quinolones ! but do I have Cystitis now? should I take a course of Bactrim? please respond and help Thanks

Hello
Thanks for your query ,based on the facts that you have posted it appears that you have increased frequency for urination with discomfort in lower abdomen .

I shall be happy if you can post me few more details to help me in making comments on your problem.
1) Do you have urgency or hesitancy along with frequency for urination .
2) Are you diabetic?

The frequency for urination and discomfort that you have could be mostly due to Enlarged Prostate .

Need to consult qualified Urologist for clinical and digital rectal examination and get following basic tests done to confirm the diagnosis.
1) Urine routine and culture.
2) Ultrasound scanning of abdomen and pelvis
3) Serum PSA.

Further treatment will depend upon result of these tests and final diagnosis.

Till then you can take Tab Tomsulosin once daily .Continue taking Ciprofloxacin twice daily with urine alkalizer thrice daily .

Thanks a lot for your input. I do have urgency and when I drink lot of water ofcourse frequency. I feel the cipro is working now ...so far 3 tabs of 500 mg each! The frequency has decreased.
Q how long should I take cipro BID ?
Q. Am I missing the DX of cystitis?

Thanks for follow up .

Since you are getting response to Cipro continue it for one weeks and repeat your routine urine test to confirm whether infection is controlled or not .

Drink more water .

Considering your age I would still suggest you to get investigated to rue out enlarged Prostate which is common at this age .
