Question: Hi Doctor.

I am having frequent vomiting with continuous abdominal pain. I am also having slight pain in my penis and burning sensation in my rectum. The Doctor has recommended Premosan tab before meals and Buscopan 10mg two tablets 6 hourly. He has advised Cipromax 500mg twice a day after 2 days if the problem persists. I feel occasional feverish but no fever. Pl advise.

Most likely you are suffering from Gastroenteritis. It is an intestinal infection marked by diarrhea, cramps, nausea, vomiting, and fever. However, it's not necessary that all symptoms are there in a single patient. Depending on the cause, viral gastroenteritis symptoms may appear within one to three days after you're infected and can range from mild to severe. Symptoms usually last just a day or two, but occasionally they may persist as long as 10 days.

Treatment-

Drink plenty of fluids to avoid dehydration–You need to drink more than usual to replace the fluids lost from vomiting and diarrhea. Water is best, but you could also try fruit juice and soup.

Take paracetamol for any fever or aches and pains.
Get plenty of rest.

If you feel like eating, try small amounts of plain foods, such as soup, rice, pasta, and bread.
Use special rehydration drinks made from sachets electral solution to maintain depleted electrolyte levels in serum.

Take anti-vomiting medication (such as Zofran 4 mg twice daily) and antidiarrhoeal medication (such as loperamide up to 12 mg a day) in case if loose stools start.

Antibiotic and anti protozoal course should be taken for 5 days.
You can continue Ciprofloxacin 500 mg twice a day(9a.m-9 p.m)
Add Tab Metronidazole 400 mg twice daily(9a.m-9p.m)
Cap Econorm twice daily(Probiotic)
Tab Meftal spas twice(Anti spasmodic anti inflammatory), or you can continue Buscopan only

Hello Doctor. Started the Cipromax tablet today as per advice from the locsl doxtor. After the first dose stomache pain was less. However, the vomiting is still there and cannot keep much after eating. Had three bouts of vomiting since starting the antibiotic. I an taking the anti vomiting tablet before earing. Please advice .

Ciprofloxacin covers just bacterias while metriniodazole covers protozoans also.Therefore a bilateral coverage is given by their combination.

Pain in abdomen is due to infection.This infection causes irritation and inflammation.So as infection will settle down(with antibiotic and anti protozoal) pain and inflammation would also settle down themselves.

For anti emetic take Emecet (ondanseteron) 8mg thrice a day along with Pantoprazole 40 mg twice d daily.Stop metochlorpromide.
