First of all I need to know since how long she has been on AKT3 medicine.
This is important because if she is on the medicine since few days only then she has to take acidity medications along with AKT3 to control vomiting.
But if she is on AKT3 for few weeks then it means she is not tolerating some component of AKT3 , in that case that component ( AKT3 contains 3 drugs) has to be identified and replaces with alternative drug.
Also there is a possibility of liver damage due to AKT3 . To check for that I recommend a test called Liver function test.

If she is on this medication for one month then it means she is not tolerating one component of AKT3.
AKT3 contains 3 drugs - Rifampicin, Isoniazid and Ethambutol. Among these the probability of intolerance is high for Rifampicin. So she may have to take an alternative for Rifampicin and continue Isoniazid and Ethambutol.
But since Rifampicin is an important component of AKT3 , I recommend her to use acidity medications like Rabeprezole for 1-2 weeks while continuing AKT3.
If inspite of this she continues to vomit then ask your doctor for an alternative for Rifampicin.

She is already taking pentadoc and emeset for the acidity part , also her liver was weak in the childhood . She has been told to switch to Akt4 now . Is that ok sir and also i hope that she would be able to convince now . Please share your valuable views on these aspects whether switching to Akt4 is ok, effect on her liver and conceive part

First of all I need to know what is her present Liver condition because if her liver is compromised she will not tolerate either AKT3 or AKT4. So I advise a Liver function test for her . Kindly get it done and revert back with report.
Now AKT4 consists of 4 TB drugs ( AKT3 + Pyrazinamide ) . Actually AKT4 is a proper treatment to begin with than AKT3. But changing over to AKT4 will not resolve her symptoms , infact it may even aggravate nausea or vomiting.
She can switch over to AKT4 but do get Liver function test for her , if it is deranged then she has to be given alternative medications for Rifampicin Component.
