Brief Answer:
Most probably Inguinal Hernia.

Detailed Answer:
Hi, with regards.

Thanks for your history, albeit short and specific.

To recapitulate: Male/78 - pain in groin - **''some swelling that comes and goes'' - taking Tylenol for pain - wants suggestions

This history of a swelling in the groin with pain - which comes and goes, the most probable diagnosis is an Inguinal Hernia.

Please get confirmed by an examination by a General Surgeon and get operated if this is hernia as it is symptomatic.
He will also help to confirm or rule out any other problem in that area.

I hope this answer helps you to get a proper diagnosis, and get a proper further treatment. Please feel free to ask for more or if you need further clarifications or if you feel there is a gap of communication.

I too thought it might be a hernia. When ever I put a heating pad on the area the swelling decreases and goes away. Also, It does not swell while sleeping. Swelling comes with activity, i.e. golf. Does this additional information add anything to your original finding? Thanks.......
