What Causes Gullet Thrush After Chemotherapy And Radiotherapy?

Question: Hello doctor. I'm a female of 64 years, and have just spent the last 6/7 months being treated for Lymphoma, described as Stage IAE thyroid diffuse large B cell lymphoma, and involved 3 infusions of R-Chop Chemo and 15 sessions of radiotherapy and am now in remission stage. I completed Rad on March 4 and have now been discharged from that dept. I'm waiting to see my specialist Haematologist for a final review, I hope. The after affects of treatment I am going through is one thing in particular; thrush in the gullet which I've had for some 5 weeks. I did have some mouth thrush and Flucozamole helped that. However it has not helped the gullet thrush. I am awaiting results for the second time of a throat swab which as of now has not yet materialised - it was done on wednesday. Oh, I also have a problem with a vocal cord palsy which was as a result of the pressure from the extra nodal tumour on the enlarged left thyroid gland. I am constantly clearing my throat as I had trouble from mucous plugging during and post radiotherapy. I'm trying to keep off dairy products as much as possible. I've been looking at health sites online to see where I'm at. I don't know if my immune system is still an issue. Can you help further please? XXXX

Brief Answer: Courage as you battle with this lymphoma. Detailed Answer: Hi and thank you so much for this query. I am so sorry to hear about this lymphoma and the current thrush you are battling with. I am however happy that you were able to go through chemotherapy and radiotherapy successfully and now in a state of remission. I hope it never relapses again. Cancer, chemotherpay and radiotherapy individually affect the immune system and, a combination of the three further worsens the immune status. This is the reason why you have developed an opportunistic infection in the form of mouth and gut thrush. This would need aggressive and sustained treatment to make sure that it gets cleared off. Gut thrush needs fluconazole for at least 2weeks. I hope your doctors have been able to do just this. If you should fail to respond to treatment appropriately, then it would be great if you could be investigated for other possible causes of pains in the throat like CMV, herpes, etc infections which are also common in persons with a weakened immune system. This would require an upper GI endoscopy and analysis of specimen/samples obtained from the throat. I hope the culture results return an give a definitive diagnosis as to whether it is for sure thrush only of thrush associated with another disease process. In all, your immune system is still very much of a concern. I hope the swab results would shed more light on the exact cause and help guide treatment. I wish you well as you continue this fight.
