What Causes Hard Lump Near Scrotum?
Question: I have a hard lump on my left side in the scrotum, near inner thigh. Size is about an inch long

Clinically, in my opinion the most likely possibility (based on the details provided) is a cyst. An enlarged lymph node is very common in the area of groin, but not so common in the area that you described. A fat lump (lipoma) as my fellow surgeon discussed is also a possibility. Chance of cancerous growth is rare - so do not be worried (if that is what is concerning you).
Get to the surgeon for physical exam. He/she can plan an ultrasound check / biopsy to arrive at the conclusion. In the meantime, do watch over the lump growth once or twice in a day.

I have confirmed via dr visit, scrotal abscess with redness, inflammation, and what appears to be induration of the epididymis.... Problem is growing. I have two days of 500 mg of antibiotic. Minus pain associated with inflammation, very little pain. Assuming this is accurate, what are the standard next steps? Age = 45. No other complications such as fever, etc

Abscess was not in my list since you never complained of pain/discomfort or the redness. However I am extremely glad you visited a doctor, surgeon perhaps and got this diagnosed.
Now, some abscess especially large ones don't respond well to antibiotics. I assume you been started on penicillin or a macrolide or a broad spectrum antibiotic. But if that isn't controlling abscess causing infection, as a physician, I would like to know the extent and plane of involvement and the exact pathogen. Clinical examination to a large extent will help us identify the plane and extent. If at the end of examination things are not clear, we can opt for an ultrasound scan.
Should the abscess be large - it should be drained either with an open incision or needle. The sample can be sent for microbiological tests and drug sensitivity tests which will then direct us towards choosing right and effective antibiotic.
Having said that, I am not sure why you feel the problem is growing? Do you mean the lump is getting larger?
