What Causes Haziness In Right Upper Side Of Lungs?

Question: My 16 month son had cold with running nose (for 20 days) and mild cough (for 10 days)

1) At 20th day, X-ray indicated right upper pneumonitis and blood tests showing WBC 15300/Cumm, Haemoglobin 9.6 g%, Lymphocytes 58%, ESR 1st hour 44mm, ESR 2nd hour 85 mm

2) He was given 300 - 300 - 300 mg of Inj Advent for 4 days
Then 450 - 450 mg of Inj Advent for 3 days
3) X-Ray indicated moderate resolution.
4) Then 5 ml - 5 ml of Syrup Advent for 6 days
5) X-Ray still showing haziness. But there are no clinical symptoms - no cold or cough.

6) The physician has now advised Montair LC Kid (4+2.5 mg) for a month
And Syrup Riconia, Syrup Rubired for a month

Is it safe to take Montair LC Kid (4+2.5 mg) for a month even if there are no clinical symptoms?

What would cause haziness in the right upper side of the lungs without any clinical symptoms after receiving around 13 days of anti-biotic treatment?

Should we be concerned and do further diagnostic tests to rule out any potential problem?
Or keep the continuing with the advised medicines?

Hi...I will come to your queries after some questions for which I need answers to guide you properly -

1. Can you upload an image of the Chest X-ray or what Chest X-rays you have?
2. Is there any family history of wheezing?
3. Was blood culture done before starting intravenous antibiotics?
4. Did your son had wheezing anytime in the past?
5. If he had wheezing in the past - were seasonal changes aggravate it?

Possibilities -
1. With the blood counts you have quoted and the symptoms reducing so soon, I feel that it should have been a viral illness.
2. Whether it is pneumonia or not can be decided only after seeing the X-ray image.

Coming to your queries -
1. Montair LC kid can be given even if symptoms are not there as it is mainly used to prevent symptoms and not cure symptoms.
2. Viral illness can cause haziness in the right upper side of the lungs without any clinical symptoms after receiving around 13 days of anti-biotic treatment.

--attached images for XRAYs taken on 13/11, 21/11 and then finally on 27/11

2. Is there any family history of wheezing?
--no history of wheezing

3. Was blood culture done before starting intravenous antibiotics?
--yes, the report was negative, also my son is not having any fever for last 2-3 months -- before or after starting with cold / cough symptoms
--also, please note that he is not having any symptom right now

4. Did your son had wheezing anytime in the past?
--at present he is comfortable while breathing
--we NEVER noticed any breathing problem today or anytime in the past
--EXCEPT some sound in the chest during deep sleep (date 10/11 to 15/11 approximately) but that problem went away post 15/11

5. If he had wheezing in the past - were seasonal changes aggravate it?
--no wheezing history


My few queries:
1. Just to confirm once again -- Tablet Montair LC Kid For 1 month -- okay for 1Year4Month child?

2. As this is winter season -- any advise on how to take care? Heard that heaters are not good to maintain temperature in the home as they reduce O2. Is there any healthy alternative to protect child inside home during winter season?

1. No need for further investigation.
2. The haziness in chest X-ray is not pneumonia, it's only a viral illness involving lungs, which in turn has caused that noisy breathing, most probably wheezing.
3. It is OK to use Montair LC kid for 1 month. This might prevent further such episodes in this winter season.
4. In winter keep him protected with woolen clothing and not venturing in cold climate outdoors.
5. Inside home too, woolen clothing is the best.

Thank you for the quick and detailed response. I would like to ask few more queries, not exactly related to this problem but they are for sure concerning the health. Hope you don't mind answering those.

1. Are mosquito repeller liquidators e.g good night, harmful for kids? Can they lead to breathing issues?

2. Why bakery products/chocolates are big no-no especially for kids and / or during winter?

3. Which fruits are advisable in winter? E.g my pediatrician advised only apple, papaya etc. Banana, orange, kiwi, pomegranate were in no-list.

4. Is it a good practice to evenly space the medicines (not dosage) if there are multiple medicines prescribed (no specific time mentioned on the prescription, only daily once) E.g take med1 at 12, med2 at 4, med3 at 8 etc.

5. If the baby is habituated to keep breastfeeding during night intermittently, can it lead to aspiration of milk (not choking) as baby could be in partial sleep? Is it a risk?

6. My son is 16 months. His PCV booster dose is delayed till now. Should we take it now after just having antibiotic treatment few days back? Also, we never gave him flu vaccine shots. Is it advisable to take? If yes, when?

Hi...coming to your queries -

1. Sometimes yes. It all depends on how the kid's lungs and airways are responding to a particular fumes of the mosquito repellent liquidators. If you have observed that he is sleeping well without a repellent on and has some breathing issues with it on - then better avoid it. You are the best judge for it. But the answer form my side will be - yes, there is a possibility.

2. This is a myth and chocolates and bakery products can be given in any season. NO relation to cough and cold.

3. All seasonal fruits are best for the baby. This also a myth that certain fruits cause cold. I will tell you a simple example, it is a general notion that oranges and other citrus are bad for cold. But you can check - actually they contain citric acid or vitamin C which is good for allergy and general resistance.

4. No need to space the medicines if the baby is able to take the medicines at a time.

5. HI...theoretically risk of aspiration is there. But practically speaking this practice leads to dental caries and recurrent ear infections.

6. Give his PCV booster and flu shot 1st dose today itself. NO need to wait. Cough, fever, antibiotic usage and cold are not contraindications for vaccinations.
