![default](/r/images/default.png)
Question: My son is diagnosed typhoid. He is 4 yrs 10 month and 16 kg. His type H and O count was 320. Doctor has put him on 750 mg IV twice a day for 4 days on ceftriaxone. His fever spike has reduced from 103 to a constant 100 and is going down daily. Today is 3rd day. It's averaging around 99 today Mon 7th Nov. But suddenly in afternoon it peaked to 104F and then in 20 mnts it dropped back to 100 and in next 10 mnts to 99. I don't know what caused this and would like to know. His malaria test is negative. After 24 hrs of no fever since yesterday 5pm it started again and is around 100.8 at 11pm today Tue 8th Nov. Given calpol. Urine culture is also fine. He is also complaining of a bit of headache and dry cough. Blood culture report is pending and will come on 10th Nov. Doctor is telling to remove from IV tomorrow 9th Nov after 4th day of IV and switch to oral antibiotics pecef 100 for next 7 days. Need to know if this is normal symptom or the treatment is fine or needs some changes.
Brief Answer:
It's normal symptom and treatment is going in right direction
Detailed Answer:
Hi....I understand your concern.
Occasional spikes of fever are common during typhoid.
The treat is going on in the right direction.
The oral switch over therapy with Cefixime will be at the dose of 20mg/kg/day of Cefixime in 2 divided doses to complete the full course of 14 days of antibiotics.

He has been removed from IV ceftriaxone and is put on pecef 50. Twice a day 7.5 ml for 5 days. He gets mild fever in the evening 100.7F which comes down on its own. He is eating well. Blood culture report confirms senstive to cefolospirin. So the medicine is fine. Are the medicines and doses fine along with duration.
Brief Answer:
Dose and duration are perfect, please follow the same
Detailed Answer:
Hi....the dose, medicine and the duration are perfect. Please follow the same.
Make sure that the total duration of antibiotics including both ceftriaxone and cefpodoxime should be 2 weeks.
As his appetite has improved, the fever also will settle in due course and I feel that he is on the recovery path.
