What Causes Heart Palpitations While Playing A Sport In A Hot Environment?

Question: I have heart palpitations and when I am in a hot environment playing tennis my heart begins palpitating rapidly and I have to slow down or stop. Why does this happen?

Regarding your concern, I would like to explain that heart palpitations, especially when triggered by a hot environment may me an expression of orthostatic hypotention.

In such circumstances, other complaints may be present, such as profound weakness, dizziness, chest or body discomfort, sweating, etc.

Coming to this point, it is recommended to closely monitor your blood pressure values at least for a couple of days, especially when heart palpitations are present.

I advise you to discuss with your attending physician for a comprehensive medical checkup.

A careful physical examination to rule out any abnormal findings (heart murmur, peripheral edema, arrhythmia, etc.), coupled with a resting ECG, chest X ray study, and several blood tests (complete blood count to rule out anemia, thyroid function tests for excluding hyperthyroidism, fasting blood glucose, blood electrolytes, etc) are necessary to exclude alternative responsible causes, as well).

If suspicion of orthostatic hypotention remains, then a tilt testing is advisable.

If palpitations persist even at resting position and become worrisome, then an ambulatory ECG monitoring (Holter) is recommended.
