1) Most common cause of FIRST trimester abortion is chromosomal abnormality. For recurrent Miscarriage, she needs to undergo some baseline investigation to rule out causative agent.
2) Following tests are needed to carried out under the guidance of Gynecologist.
- Test for Anti-phospholipid Antibody Syndrome (APAS).
- TORCH profile to exclude associated infection (if any).
- Endometrial biopsy to rule out Luteal Phase Defect (LPD).
- Test for Ovarian Reserve (Anti-Mullerian Hormone assay).
- Karyotyping of conceptus if miscarriage occurs in next pregnancy.
- Blood test for thrombophilia includes factor V Leiden, Prothrombin, Protein S.
- Serum TSH, Progesterone, Blood sugar (PPBS), Hb%, Blood Group & Rh Typing.
3) Both of you take healthy diet with more green vegetables and fresh fruits, vitamin and Folic acid supplement for at least 3 months, control body weight, avoidance of salty/ fatty/ high calorie foods, drink more water or fruit juice, maintain genital hygiene, avoid mental stress and need sound sleep. These are essential to build a healthy life style.

My wife was told by a doctor after USG that she has hemorrhagic follicle in right ovary. She has extreme pain on the right side usually of the abdomen. Is there a possibility of any infection? is there is possibility that It would spread any infection. She has extreme abdominal pain and it radiates down the legs. She also has lost her libido.

According to your description, it is MOSTLY suggestive of hemorrhagic ovarian cyst (chocolate cyst) and occurs during ovulation time when the follicle doesn't release the egg. It is associated with endometriosis of the ovary.
They usually do not burst. However, they do leak blood and result in searing pain throughout the pelvic region. It can lead to irregular periods, leg pain and even pain during intercourse. Infection may occur if it is left untreated.
If the cyst grows abnormally large or bursts, the pain will increase substantially. Follow up ULTRASOUND SCAN is required to assess the nature of the cyst and next plan of treatment can be made according to that.
Birth control pills are usually prescribed to control hemorrhagic cysts. If the pain persists continuously, your doctor may suggest cyst removal laparoscopically.
To summarize, I suggest to consult with your treating doctor or gynecologist regarding this for proper management. Until then, she would take painkiller like Aceclofenac with antacid (Ranitidine) twice daily to reduce pain.
A warm bath, heating pad/ hot water bottle applied to the lower abdomen near the ovaries can relax tense muscles and relieve cramping, lessen discomfort and stimulate circulation and healing in the ovaries.

1) She would take PAINKILLER like Aceclofenac or Ibuprofen with Antacid (Ranitidine) twice daily to reduce pain. It will be helpful for immediate purpose.
As I have mentioned earlier, she would use heating pad or hot water bottle to the lower abdomen near the ovaries to relax tense muscle and relieve cramping, lessen discomfort and stimulate circulation with healing of the ovaries.
2) I suggest you to visit emergency department of nearby hospital if her abdominal pain is SEVERE and UNBEARABLE. She may require urgent medical or surgical treatment.
Otherwise, you can visit to your GYNECOLOGIST tomorrow morning if pain is moderate degree and reduced after taking PAINKILLER.
