What Causes High Cholesterol Levels With Normal Angiogram?
Question: Hi,
My father os 52 year old now and recently he had high blodd pressure in the range of 150-160/90-110. And then we met a cardiologist who prescribed lipid profile screening/ echo cardiogram/ ct-angiogram. I have attached the results. The doctor said the blood cholesterol is really high but angiogram seems normal. I request you to kindly go through the attached reports and provide your expert opinion.

Triglyceride is very high / level greater than 500 is called very high
A full 14-hour fasting is necessary for proper interpretation – did he fast properly?
Obesity / Diabetes / Thyroid insufficiency / Alcohol / over eating / some drugs like Beta-blockers, Diuretics are some causes.
It is generally genetic [inherited /running in family]
Eat fewer calories / Limit natural sugars:
Eat small, frequent meals and do not skip meals
Avoid late-night snacking
Add omega-3 fats in your diet
– fatty fish (like mackerel, salmon, sardines, tuna) twice or more in a week
– soy foods, canola oil, flax seeds and walnuts / limit potatoes, yams, beans, corn and peas
A good dietician can guide further... need to discuss a few times to grasp the subject well and follow it up
Lose weight if he is overweight
Do regular physical activity – at least half an hour a day / 5 days in a week
Because of such high level, drug should also be started simultaneously – usually in such situations a FIBRATE (Like fenofibrate or gemfibrosil) is started .
Cardiologist / Physician treating him will give appropriate prescription
Repeat the tests in 2 months
LDL is very good
HDL is a little low – not an issue now
Minor changes in ECHO are from pressure / heart function is good / Nothing to worry

Hi doctor,
Thanks for the reply. No there was no instruction to fast. Would you recommend taking another test with 14 hr fast this time. And the doc prescribed below medication.

Zivast is Atorvastatin / statins are good, but have more effects on cholesterol than Triglyceride(TG). Fibrates have better effects on TG. I will add a fibrate; for example change to something like Zivast-F that has both Atorvastatin and Fenofibrate
Repeat the test with 14-hour fast
Monitor the pressure; if the pressure isnot controlled in a week or more, the dose can be increased or a drug of the class ACEI (Angiotensin Enzyme Inhibitor) ARB (Angiotensin Receptor Blocker – like sartans) can be supplemented.
