What Causes High Fever, Swollen Lymph Node, Throat Pain And Occasional Chills?

Question: My father, aged 53, residing in XXXXXXX has high fever (101- 103.9) since 6 days. Associated with throat pain and occasional chills. 1 single left cervical anterior lymph node is enlarged but XXXXXXX No other symptoms. Blood parameters- hemoglobin, wbc, and electrolytes are Normal. ESR is 22. BUN - 15, creatinine- 1.2. Malaria, widely test and dengue negative on 2 occasions.
X-ray shows increased bronchovascular markings. He is a known hypertensive.
Currently on cefexime 200 and azithro 250 since 2 days+ paracetamol since 5 days. What can more be done?

Hi, I am Dr Anshul Varshney , I have read your query in detail , I understand your concern.
Your father seems to be suffering from Streptococcal Sore Throat. Or this can be Ebstein Barr Virus infection as there are swollen lymph nodes associate.
Antibiotics like Cefixime and Azithromycin have good coverage but when swollen lymph nodes are associated i trust Amoxycillin with Clavulinic acid (Augmentin) more.
I advise as following in such scenario:
1. To take an antibiotic like Amoxycillin/Clavunate .
2. To take an anti-allergic like Levocetrizine, chlorpheniramine.
3. To do warm saline gargles.
4. To do steam inhalation
5. To take Paracetamol on as or when required basis for fever and body pains.
I am hopeful once this treatment is initiated, he would respond with 48 hours.

He was on augmentin and the same regimen for 1st 4 days. But no response. Then he was put on cef and azithro. Still no repsonse. Dolo is being taken 3 - 4 times a day. We even tried mefanemic acid Still no repsonse .

Get a Complete blood count done

These days the infections which are viral with secondary bacterial infection are taking up to 7 days to show improvement. In fact, we are continuously encountering this problem in our day to day clinical practice specially in XXXXXXX and NCR. In case if his location is the same.
Now, since he has been started on Cefixime and Azithromycin a day before, you should wait for next 48 hours.
You should also get his Blood culture and Complete blood count done (a fresh one) to check for any infection that might be ongoing.
His fever should get better in next 48 hours and even if not, he would require a complete workup and evaluation.
