What Causes Increase In CA-125 After Chemotherapy?

Question: My wife age 50 years had overian cancer and treated in XXXXXXX and uterous,overian were removed and six cycle of chemo given which was completed in March 2015. The CA125 count which was more than 600 before surgery and after each cycle of chemo it has reduced to 390, 270,90,35.2. However after 3 months ie XXXXXXX 2015 again CA 125 count has increased to 922. Doctor advised to take VOTRIANT 400 mg.
which she is taking from 15.6.2015 but now the appetite reduced and always she feel stomach is full and I feel by touching stomach it is rough. Please advice how to improve the appetite and to control the stomach pain and to reduce CA 125

Thanks for your query. I understand your concerns.
It appears that CA 125 has started rising soon after completion of chemo. Before starting votrient, have we done any CT scan to check if there is any disease visible? Because in that scenario, votrient is not a good drug and chemo is preferred. I am afraid data for votrient use is limited to use after completion of initial chemo to keep the disease in check. When the disease has come back, its role is less.

If a CT was not done before this, then it should be done now. These symptoms may be due to disease. Appetite may be reduced because of votrient but pain is less likely. We can try to boost the appetite by meds like aptivate syrup or megestrol tablets. You can talk to your doc regarding these. for pain simple pain killers like paracetamol or tramadol may be enough.

Hello Doctor,

I have uploaded the pre CT Scan reports and also a gist of question along with the present one.
The US Scan taken on 13.06.2015 shows only fat has accumulated due to the surgery.

Please see the reports and advice please.

From these reports it appears that the disease has unfortunately become chemoresistant. In case of relapse free interval of 6 months or less the disease is very aggressive and pazopanib alone is not sufficient to control it. Ultrasound is not enough to assess disease status. We need a CT scan.

If that shows disease we need to discontinue pazopanib and start some new chemo like liposomal doxorubicin +/- avastin. I suggest you take a DM medical oncology doctor's opinion in your city.
