What Causes Increased Blood Pressure After Switching From Amlodipine To Telmisartan?

Question: I was on amlodipine 5mg and bendroflumethiazide 2.5mg for a 3year hbp. I hardly slept more than 3 hours but my blood pressure was controlled at an average of 130/95.

I went to see another doctor, he changed my medication a week ago to co-micardis ie telmisartan 80mg/12.5. Immediately I started sleeping well however I've noticed that my blood pressure even as soon as I wake up with this new prescription is 150/101.

Should I immediately go back to amlodipine?

Regarding your concern, I would explain that your actual blood pressure readings (150/101) are pretty far from being optimal.

First of all, we need to wait for a couple of days, as Telmisartan may need a period of 2 to 4 weeks to fully show a blood pressure profile improvement.

Meanwhile, you should closely monitor you BP values (several times daily) and write them down.

If after that period your BP values have not shown adequate improvements, then it is an urgent need to optimize your antihypertensive therapy. Adding Amlodipine to Co-Micardis, as well as any other antihypertensive agents would be necessary. Finding an efficacious and safe - profile antihypertensive scheme is usually a matter of trial and checking.

All the above therapeutical steps need to be strongly supervised by your attending doctor.

So, my advice is to contact your prescribing doctor again and have a valuable discussion about the above mentioned options.

Is your opinion to continue with Telmisartan even though I feel very dizzy and unwell ever since I started the tablets as opposed to amlodipine?
I could also have a glass of wine in the evenings with amlodipine but with this the couple of times I tried it I felt terrible?

I am in another country and it will take another week or so before getting back to see my original doctor who prescribed the amlodipine, is there any harm in going back to amlodipine until I get to see my doctor again?

You can safely use Amlodipine.

Your blood pressure is not within normal ranges, so it is reasonable you feel dizzy.

I would not recommend you to take wine in the evening, perhaps during the lunch it would be better.

Returning to your last question, I would say that, there is no harm using Amlodipine in addition to Telmisartan. So, you may have at least 5 mg Amlodipine daily, and check your blood pressure repeatedly.
