Posted on Mon, 23 Feb 2015
Question: For the past two weeks I have been experiencing increased discomfort in my lower to mid back on the right side and the lower abdominal tenderness on my right side aswell. I have no fever, no problems urinating or any problems with bowel movements. I tarted to notice the issue while drinking a daily smoothie with, spinach, XXXXXXX berrys, banana and spurilla. Irecently visited my doctor on January 9th of this year and he did a physical exam on my stomach area, no abnormal findings . I had blood work done in September of 2014 and everything was normal for both kidneys and liver. I have been thinking I had a possible kidney infection and have been using cipro for 3 days and it seems to have helped although im not sure if its mental. I also have awaken around 5am three times in the past month to terrible gas in my adomen and takes 5-10 mintues to pass, I have gained around 30lbs in the past year bulking up. I did have eleveated liver enyzmes in May of 2013 but since have been tested twice and they are now within normal range

I also started doing a difflent movement while weight training that involves a lot of lower back and core strength around the same time as the symptoms showed up. I notice that some nights heat will help and some nights it wouldn't.

The pain level was around a 7on a scale of 1-10 until I started taking cipro, it is now around a 2 and consistently getting better.

forgot to add, that I were I am am a little tender from the belly button around to the back on the right side and the later in the day it gets the more it seems aggravated vs early morning and mid day

One more thing to add... I noticed today working lifting heavy at work that the pain has comeback. I strong back rub made it feel better for a bit but has moved to the left side of my back aswell

Brief Answer:
Investigations to get the exact diagnosis to get a curative treatment.

Detailed Answer:
Hi.
Thanks for your query and an elucidate history.

To recapitulate for my sake :: 34 / Male - 2 weeks - increasing discomfort - right side - lower to mid back and lower abdomen - noticed while drinking daily smoothie - thought of kidney infection and took Cipro for 3 days which helped - gas awaken thrice in the last month at 5 am - takes 5 to 10 minutes to pass - gained 30 lbs in last year- all blood tests normal now - weight training since the same time - heat helping sometimes only - Pain level dropped from 7 to 2 after Cipro and consistently getting better - but the pain gets aggravated as the day passes by - today > heavy lifting - increased the pain relieved a bit by back massage but spread to the left side of the back ...

After correlating all the history you have provided, I think of the following:
This may in fact be a combination of kidney infection as Cipro has helped you so much. Since the kidney is at the back side of the abdomen and you are also having the Back pain which might have accentuated by the kidney infection, daily chores, exercises and now heavy lifting.

I would advise you the following:
-Consult a Doctor again, give detailed history so that He can do an actual physical examination, advise you the investigations as per his clinical findings and treat you accordingly.
Blood: if your Doctor feels so as you recently had got this done, yet WBC count to see if it is increased.
Urine : Routine, microscopy, culture and sensitivity.
Ultrasonography of the abdomen
MRI of the spine - back and for kidneys

These tests will help to get a proper diagnosis which will give the proper guideline for the treatment to get a cure.

I hope this answer helps you, please feel free if you need further clarification or if you feel that there is a gap-of-communication.

thanks for the response doc, I started to notice that the cipro really has worked as much as I had though, upon lifting at work again I noticed the pain came back. I went to a chiropractic this am and upon an adjustment I have felt instant relief. Now I have very little discomfort. Is it possible that it was a back injury?

Brief Answer:
Continue Cipro for 2 weeks.. Yes it is possible to be a back injury

Detailed Answer:
Thanks for your appreciation.
If Cipro is giving such a good response, it is better to continue for 2 weeks; take plenty of oral fluids.

This is what I was thinking and suggesting of, it is possible that there may be a primary or secondary back problem as already discussed above. It is a good news that you got an instant relief from Chiropractor.
