Hello! Thanks for putting your query in HCM. I am a Gastroenterologist (DM). I understood your concern. I will suggest you to:
1. Avoid fatty meals
2. Don’t lie down soon after meals for at least two hour
3. Take small and frequent meals, instead of full meals at a time
4. wear loose clothes
5. If you are overweight or obese do weight reduction with dieting and exercise
Among the medications:
1. I would suggest the use of levosulpiride. It don't simply act as a prokinetic agent but it also increase the tone of lower esophageal sphincter
2. Baclofen also helps in increasing tone of lower esophageal sphincter
3 Instead of protonix, I suggest ilaprazole which has better efficacy, and I found it very useful in my patients
4. Also add tablet famotidine at bed time
For loose stools take a course of probiotic and tablet rifaximin as patients who take proton pump inhibitor for long time may develop bacterial overgrowth leading to diarrhoea
