What Causes Increased Urea, Creatinine And Potassium Levels?

Request for your advice on following. Many Thanks.

Background
My mother is 67 years old. She is diabetic (type 2). In 2008 she was detected with multiple blockages in heart and had a undergone bypass surgery in 2008. She was on allopathic medicines since then.
For last 2 years she had to be admitted in hospital after every 4 -5 months (rushed to hospital in emergency) with complaint of breathlessness. Doctors say that her heart strength is low (LEF at 20-25%) , she goes into heart failure and as a result water gets filled in her lungs resulting in breathlessness. For last 6 months she also suffered from angina pain .
A month back (16 Aug 2014) she suffered minor brain stroke (right brain) impacting her left hand (mainly) and leg. Doctors say most likely the root cause was low functioning of heart.
Due to high intake allopathic medicines and low water intake – as suggested by cardiologists – her kidney also got impact showing high levels of urea, creatnine and potassium. She has to be admitted to hospital for high potassium level 6.86 (6 Sep 2014) . Last week the parameters were normal (Potassium 4.5, Urea 20 and Creatnine 1.1) however two days back the levels were Urea 61, creatnine 1.3, and potassium 4.7.
For sugar she was on insulin however since her kidney parameters were normal diebetologist put her on medicine from 27 Sep onwards. From 27 Sep 2014, we started Glysiphage 500 and trajenta 5 mg. however, after recent blood results (1 October) doctor changed it to Trajenta 5 and Glynase 5 (breakfast) + Insulin if required. Though he was of opinion that metformin does not impact kidney. He also suggested to taken Nefrosave for two weeks.

Medicines
We are taking following medicines

Acitrom 1 mg (1 time). Since INR was 1.47 it has now been recommended to take 2 mg after 1 days
Levipil 250 (2 times). Reduced from 500 mg earlier to 250 in last 2 days

Nitrocontin 2.5 ( 2 times)
Flavidon mr ( 2 times)
Carca 6.25 – ( 2 times)
Thyronorm 75 mg (1 time)
Febumac 40 (1 time)
Valent 40- ( 2 times)
Ecosprin 75 Av – (1 time)
Ecosprin 75 Av – (1 time)
Dexorange and nuhenz (1 time)
Nefrosave (2 times). Started 2 days back.

Questions
Please suggested if there is any kidney problem that need to be addressed or its only impact of medicines which is resulting in increased urea, creatnine and potassium levels. As album results were okay. If so then what is the solution to avoid it.
Are there any medicines or precautions that we should take. Is it okay to take nefrosave.
She is also taking homeopathic medicines. Have there been any cases where homeopathic medicines have impact of kidneys?

Your mother has multiple risk factors like Hypertension , Diabetes and Less functioning heart , any 1 of them can cause damage to the kidneys.
But a mild elevation of urea or creatinine doesn't indicate kidney damage. In fact in the nephrology discharge they have mentioned her as patient of CKD . CKD means Chronic Kidney diaease.
As far as I know any patient with CKD will never have a normal creatinine value which your mother has . So she is not a patient of CKD.

As much as I feel that the reason for her elevated creatinine was because of her fluid restrictions . I am not saying that she can drink any amount of fluids but her allowance for fluids has to be increased by 200ml extra per day.

I believe that there may also be a role of Homeopathic drugs affecting her kidney. I have come across 3 cases where we could not find any reason for kidney damage but once we stopped homeopathic drugs kidney recovered completely.

There is no problem in taking Nephrosave .

Dear Doctor

Thanks for your response. Is there any other medicine that we should take to protect or strengthen kidneys. Our doctor also mentioned about diabetic nephrology I am not aware about it, is she seems to having it. Pl suggest course of action.
Will take care about water intake.

HI

I am reassuring that she is not having any kidney problems at present .

What you doctor has mentioned is called Diabetic Nephropathy , which is a slow damage to kidneys due to Diabetes. The main thing in the prevention is good control of diabetes . Also she has been started on Valent 40 which helps in protecting both Heart and Kidney . But this drug increases Blood Potassium levels , so Potassium levels has to be checked every 3 months.

So make sure her Diabetes and BP are controlled well . Also get Urine Microalbumin test done and if it is raised ask your doctor to start Taurine supplement. If it is not raised keep repeating the test every 6 months.

She has also been started on Febumac which is a medicine to reduce Uric acid levels. Kindly ask your doctor to evaluate for the cause of raised Uric acid .
