Question: Problem is intense back thigh pain when rising from a sitting position. Both legs. Not a sports injury, older male patient. Does this suggest any more specific diagnosis?

The back thigh pain while getting up with no history of injury to the spine is likely to be due to a disc prolapse or a muscle spasm.
Other causes include
Spinal stenosis
Osteoporosis
Fibromyalgia
If its unbearable take medications or muscle relaxant prescribed by your physician. And if it has been long standing illness visit your orthopaedician and clinically-radiologically establish a diagnosis to commence definitive treatment.

It doesn't feel like sciatica, (been there, done that). Both legs, not at all one-sided. Very crampy, muscles become very hard. It's very difficult to walk after standing up, for several minutes. The cramps eventually subside but ache remains, seems deep in bones. This is not a long-standing condition. I'm moderately active though that is becoming more difficult. I don't think I have a big osteoporosis problem. Fibromyalgia, who knows, but none of those seem quite like a fit. We were hoping these symptoms would point toward a more specific diagnosis so I'll be able to choose a more specialized doctor, rather than medicate the symptom only. Are there any other less common conditions that might be a possibility.

From the description s muscle spasm seems more likely that can happen due to fluid and electrolyte imbalances or hypothyroidism or Vitamin B 12 deficiency. You can get the blood tests done and in case any problem is detected start with proper supplements. Stretching exercises for muscles will also help.
