Going by your symptoms i suggest that kindly visit a cardiologist and a consultant neurologist for the above complaints.
Since you have a history of heart block and you do complain of chest pain on and off, you need to get a thorough cardiac evaluation :
1. ECG preferably a holter ECG(24 hour constant ECG monitoring) to detect any atrial fibrillations.
2. Echocardiography to screen the mechanical functioning of the heart.
Since you are complaining of weakness and twitching it is important that you may need further evaluation by a neurologist for screening out symptoms of stroke . TIA Transient ischemic attack is a warning of a bigger stroke event that can occur in future hence it is very essential to be treated early as possible so that you can protect your self from bigger events.
However, please stay calm , donot worry.
Appropriate neurological tests are required like:
1. Carotid Doppler ultrasonography
2. MRI scan
3. Magnetic resonance angiography
These investigations shall definitely prevent any bigger events in future thus reducing the chances of stroke.
Also your Blood Pressure needs constant monitoring and it needs to be kept under control.
However,as of now since you are complaining of tiredness please consult a neurologist at the earliest.
