Brief Answer:
Wait and watch

Hi
Welcome to the HCM
I have gone through your question and understand your concerns but don't worry. The symptoms and of your nephew are more in favour of a viral illness. I would recommend you to wait for hep B and C reports. If they are normal then wait and watch as it may be a self limiting viral illness which will improve within one more week.
If the symptoms persists thereafter along with poor appetite, weight loss and low grade fever episodes then do evaluate for tuberculosis as it's rampant and can give similar presentation.
Hopefully this will help you. I would be happy to help you in any further questions.
Take care
