Hello again

The number of kidney cysts and their size needs to be known before labelling a person with polycystic kidney disease.

Renal function tests should be obtained. Liver function tests including ALT should be done to look for possibility of Non alcoholic fatty liver disease or steatohepatitis. If in addition there are features of metabolic syndrome then the comorbid factors should be addressed.

GERD can be treated empirically , if clinically relevant, with PPIs and pro kinetics for a 2 week period. The anal itching should be worked up. Local exam and DRE (Digital Rectal Exam) should be done to look for possible hemorrhoids, fissures etc. Stool for ova and parasite can be advised and ultimately a proctosigmoidoscopic exam if no other cause is found.
