What Causes Intermittent Pain In Stomach Radiating To Back Area?

Question: I have periodic pain that starts in my stomach and then rolls over to my back all the way to the top and even in my shouldler. what is this and what I do? The pain is unbearable.

Hi.
Thanks for your query.
History of normal ultrasonography, consultation with a Gastroenterologist and periodic unbearable pain - that starts in the stomach- rolling over to back - all the way to the top - even in the shoulder noted - in a female patient who is 72 years old... noted.
The query is about: what is this and what I do?

I would take this in the following way:
As you have already been seen and examined by a Gastroenterologist and the ultrasonography is normal - rules out many diseases of the liver, gall bladder, pancreas and so on which can give such symptoms that you have.

Referred shoulder pain can be due to cardiac reason or from the under the diaphragm structures. Referred pain to the back can be due to retroperitoneal structures like stomach, pancreas and so on.

The possible reasons I can think of are as follows:
First of all you should get your ECG and cardiac check-up done preferable by a Cardiologist as this can be an atypical presentation of Angina or so.

Upper GI Endoscopy to rule out the problem of the esophagus, stomach (Gastric) in particular like ulcer / cancer.

MRI of the spine (whole spine survey) as some spinal conditions can mimic this type of pains. This MRI can be the best investigations for the upper abdomen solid or cystic organs (MRCP) below the diaphragm.
