Brief Answer:
Melasma: various suggestions and guidance.

Detailed Answer:
Hello,

Welcome again and thanks for posting the pictures and details. After analyzing the query I can get that your wife has the condition known as Melasma.

To tell you simply about this: it is a condition of hyperpigmentation occurring mainly over cheeks and sometimes on forehead too in superficial or deeper pigmentation. It is considered to be a hormonal problem due to unknown causes but hormones and ultraviolet light exposure are most common known cause of aggravation.

It is most commonly seen in females due to hormonal upsurge and down surge more commonly after pregnancy or at young age.

If I were her treating dermatologist then would have prescribed a cream containing Kojic acid with arbutin and mulberry extract on one night and another cream containing Licorice extract the other night.

All these creams are to be applied in a very thin layer at night time only so that these should not be visible to naked eye. Apply at night time before sleeping. A pea size amount is sufficient to be applied to palm size area.

The most important of all is to use good quality sunscreen. It is of utmost importance and is also a must in day time. In my patients I suggest them to use for 3-4 times a day. It is should be used half an hour before going out. Say for if going out at 9 am then to be applied at 8:30 am and then re-applied every 3 hourly for 2-3 times.

A good nutritious diet containing lots of fruits and green vegetables is also very helpful.

For the part of greying of hair: Do let me know who is affected with Premature whitening. Time period since the problem and how many of hair are grey.

Brief Answer:
For grey hair at 30 years, if few strands are present then is quite normal.

Detailed Answer:
Dear XXXXXXX

Thanks for your further follow up. In my patients I generally prescribe Kojic acid with arbutin and mulberry extract (Clearz Ultra) on one night and another cream containing Licorice extract (Melaglow) on the other night.

For grey hair at 30 years, if few strands are present then is quite normal. But if on higher side then improve the diet.

Doctor thanks for prescribing the medicines, one last query for how long the medicines have to be applied and if it is stopped then what will be the chances of reoccurrence

Brief Answer:
The creams suggested contains safest of the ingredients.

Detailed Answer:
Dear XXXXXXX

The creams suggested are one of the safest creams to be applied. Most of the times I see that a steroid preparation is applied by the patients. In your wife’s case too Kozimax is quite safe but on the other side Melalite XL contains Mometasone in it. This is basically steroid and should either not to be used or if used then only under dermatological guidance for few weeks only. If applied for longer time then can cause lot of side effects.

The creams suggested contains safest of the ingredients and therefore can be used for multiple of months. Once the desired effect is present then one can use it on alternate days and then for 2 times per week.

Melasma is known for its recurrence and therefore I always prefer to give the safest of the medications which can be used for longer time.
