I have gone carefully through the details and understand your concerns. While you were 16, you probably got epididymitis or epididymoorchitis which could have been caused due to certain viral infections like measles. The doctor probably just gave you pain medication to take care of the issue. I understand that the problems are recurring and involving the testis of only one side. But that can happen. Use of T-bandage or scrotal support should provide relief from the discomfort. You may also put a pillow or rolled up towel under your scrotum to elevate your scrotum when you sit or lie down. Use pain medications like aleve (naproxen) as required. Ice can help in decreasing swelling and pain if it is there. If that does not help, I think that you should get yourself examined and evaluated by a doctor.
Since you already have a kid, it is clear that you have no issues in impregnating your partner. It is less likely that you would have any issues in the future as well.
I would not really bother about the itchiness. It is probably due to a fungal infection and the use of clotrimazole powder should provide relief.

I doesn't have any pain or had any pain all this years except at beginning of that incident period.
Major concern is my "left testicle" which looks very small at this moment feels like almost disappeared in my sac, unless I really want to feel i have to search for it . Is it possible to regain to my original size with medicines Or have to perform only surgical procedures?
I can see little swollen in my left scrotum but its not paining, I am scared about Is my testicle damaged? Will it be removed? Is any health related problems occurs in future?

The cause of the atrophy of the left testicle needs evaluation. I would insist you to get a ultrasound along with Doppler study of the testes and scrotum done. It will give details on the present scenario. However, I doubt if anything can be done to improve the size. Since you have already impregnated your partner at least once, I doubt if there is anything to worry but the ultrasound and Doppler would make things clearer whether anything needs to be done and whether there is something worth worrying about.

as you mentioned in previous email i may have suffered from orchitis .but i left untreated since long time . Is my testicle is damaged completely? Is any danger with life threatening? If i feel my left testicle i can feel testicle like small peanut. If blood flow is stoped since my incident happen to till know almost 15years .if i show to urologist what kind of results i can expect? Will my blood flow can be resume normal? If testicle is damaged are they going to remove? Please advise iam very nervous and scare

I would strongly suggest you not to panic. Even if I assume that you had suffered from orchitis, the infection is likely to have subsided on its own. Whether it was infection or something else needs evaluation and I have already suggested the means to do so. I would suggest you not to jump to conclusions. Just get the Doppler USG done and based on the findings I would perhaps be able answer the rest of your questions. Thereafter you can get it checked by a urologist. I do not think that your testis needs removal.

I have consulted urologist and explain my history then he have done physical examination of testis and conformed me that I had testical torsion when I was young. Because of torsion my left testis is shrunk due to no blood supply and testis is dead. And I have asked him weather to leave dead testis inside Scotrum or it will be removed he said no need to remove as its doesn't cause any harm to you. And even told me there is no need of ultrasound also. My concern is is it safe to leave dead testis inside scotrum ? And urologist checked my groin area near lump node he felt very small hernia and he said that it's fine there is no problem with that as it is very small no need of surgery please advice me your concern weather is it OK to leave or do I need to see another urologist? Please share your concerns

Although I did not discuss with you, I had torsion of testis in mind and I feel that your urologist has given the correct explanation for the small sized testis. I do not think that there is any cause of worry in leaving it there in the present condition. I do not think that you need to see another urologist for the same and feel the explanations of this urologist justified. Let me mention that some other urologist might suggest you to get rid of it on assumption of a possibility of subsequent cancerous transformation although I doubt whether you need to worry about such a thing.

Gangrene does not occur after a long gap

That stage has already been crossed. Gangrene occurs when cell death occurs due to blood supply or oxygen gets cut off. But it occurs without delay after the insult. It is not something that occurs years after the oxygen deprivation. Instead, there is another form of cell death called apoptosis where a crisis is met by selective elimination of cells leading to atrophy. Since your testis is long atrophied, you need not worry about gangrene. Had a gangrenous condition occurred, it would have been necessary to remove it surgically.
