Brief Answer:
Cold urticaria most likely.

Detailed Answer:
Hi

Thank you for asking HCM

I have gone through your query.Your problem is most likely an allergic reaction to cold or cold urticaria. It is a histamine reaction while exposing to cold stimuli.Diagnosis can be confirmed by cold allergy test by allergist.
Better way to prevent that is avoid exposure to cold or warm up as soon as you expose to cold.If you can prevent from exposing to cold then antihistamine medications like Levocetirizine or ebastine will be helpful.
For preventing from such attack i give combination of levocetirizine and mast cell inhibitor monteleukast.
Hope this may help you.Let me know if anything not clear.
Thanks.
