What Causes Itchy Genital Rash Which Burns And Weeps?
Question: I have an itchy genital rash which burns and weeps when at its worse. I also have very dry flaky skin around my ears. My doctors are not sure if it's fungal or psoriasis/ eczema but it is hard to treat. I have diprobase and eumovate creams which give some relief but it's still very uncomfortable and my chemist recommended a medicated anti dandruff shampoo.... They do not know what the actual condition is so don't know how best to treat it. ...
Have you any ideas please? Many thanks.

I have taken a picture of genital area but I'm not sure how to send it to this site so I'll try to send it to your email address.
I'm alone today so difficult to photograph my ears unfortunately.

Brief Answer:
Looks like early lichen sclerosus of vulva
Detailed Answer:
Dear XXXX
Looks like early lichen sclerosus of vulva to me.
I would like to confirm it.
You will need to get a small biopsy of the area and also a swab culture .
Once the report comes as positive then we need to start on medications accordingly.
Do you have any white discharge per vagina?
Do you have burning on passing urine ?
Did the ear changes start at the same time as this genital rash ?
Do you have history of autoimmune diseases ?
Did you have intercourse (especially unprotected ) with a new partner ? Does your partner have any lesions ?
Do you have scaly plaques on rest of your body and do you have pitting of nails ?
It is definitely not candida and most probably not psoriasis either.

Thanks very much
In answer to your questions.
I do not have any white discharge or burning of urine at present.
The ear changes started after the genital problem. I also occasionally have a mild facial and neck rash (I have been quite stressed lately).
the itching and burning first started some time ago around a few piles I had on my anus and then spread to my vulva.
The answer to the other 3 questions is no. I don't have any autoimmune history or other symptoms/nail problems and have had only one sexual partner for 30 yrs who has no problems fortunately.
I hope this information helps
How would I go about getting a biopsy etc please?
Many thanks

Dear ,
Detailed Answer:
Dear ,
You have to go to a lab or a hospital and ask for "biopsy of vulval skin for lichen sclerosus".
If they do it they will understand immediately.
Otherwise even a dermatologists clinic should be able to do it.
Please do let me know the reports.

Dr. Som

Thanks doctor
I'll get that done asap.
Just one more point. I do not have any visible white spots as mentioned in the symptoms I've read online. Mine is more of a angry red rash. Would that be the early stages of the condition?
If I ask to be tested for lichen sclerosus and it's negative, would a biopsy be able to diagnose what the problem is?
I will let you know the results when I get the tests done.
Thank you very much for your time Dr XXXXXXX

Yes that is why i told that it looks early stages of the disease. Ideally it should become white after some time.
Its my clinical suspicion. I might not be right but the biopsy will definitely tell if its lichen sclerosus.
And even if its not it should tell what is it then . An answer should come out of the report.
