Turning of only one foot blue once in awhile indicates that some pressure is causing this as mentioned by you holding or when he is in walker. But why only one foot is a reason of concern. He needs a examination to see any local problem in hip and limbs and also examination or palpating of blood vessels of lower limb to rule out any reduced blood supply.
One moe thing , how long does it stay and what you do when you recognised it is blue , if you notice it is due to holding and it goes away after your loosenening of limb , it is most likely due to pressure .
