What Causes Loss Of Appetite, Fatigue, Diarrhea And Abdominal Pain?

Question: Dr. XXXXXXX Berger-Durnbaugh
Hello,
I have some new symptons. I have lost appetite for food, fatigued, and pain in my abdomen area. Early in February i had eleveated enzymes AST 57 ALT 64. Thru an ultrasound was diagnosed with fatty liver. The following 4 weeks i excercised daily and very liver friendly foods and stopped drinking alcohol. I felt great and was told my enzyme levels were not to much of a concern. After the 4 weeks of healthy lifestyle, Over the course of 5 days i had some beverages and wasnt eating quite as well. So i got right back into regimen. I now have these symptoms ive never had before that i mentioned earlier. I assume this is a major cause for concern??
Thanks

Brief Answer:
By beverages, do you mean alcohol?
Detailed Answer:
Hello -
Some questions first:
When did the fatigue and loss of appetite start?
Where is the abdominal pain located and how severe is it? Also what kind of pain - stabbing, aching, cramping, etc?
Does the pain occur after eating or is present at all times?
Any fevers? Diarrhea?
How much alcohol did you drink each day?
Information to follow...

Yes alcohol
Fatigue and appetite started Monday am. Couldnt even finish a bananna tonight
More of a stabbing pain
Seems present off amd on all the time
Diarrhea, yes
Probably a total of 20 in 5 days
Did i totally screw myself here? I felt so good. I had ran/jog nearly 3-4 miles everday for 4 weeks while not dronking at all and eating very well.
I know i did this to myself but im pretty comcerned and scared.

Brief Answer:
I doubt that this is acute alcoholic hepatitis
Detailed Answer:
Since your liver enzymes were never extremely high, I doubt that you have acute alcoholic hepatitis now. Given that you also had or have diarrhea, it is more likely to be a viral gastroenteritis. I think a CBC and liver function test may help sort this out, so consider calling your doctor's office and explaining what happened, ask if you can have these tests.
You were doing so well, and I know you are very motivated to get your health in order. But this is a long term sort of thing, and I am thinking it would be good for you to get help with what may be alcoholism. Your intentions are good. But alcohol has a mind of it's own if you are an alcoholic, alcohol is very hard to stay away from then. I am less concerned about the current abdominal symptoms than about the long haul.
I think if you can get into an alcohol abuse recovery program you will be on your way to having a lifelong healthy liver. Your enzymes were never extremely high, like in the hundreds. So likely you are not overly sick from the 5 days of alcohol use. But looking into the future, you may become sick from this sort of thing.

But these are symtoms of something worse right? The abdomen pain is all in the same area on the right side. Is cirrhosis the only option in regards to my liver?? Are there other stages of liver disease after fatty liver that are not as serious as cirrhosis that could be causing. Im certain this is my liver related.
The diahrea is not frequent in fact i rarely go and when i do it is mainly gas.

Brief Answer:
Probably symptoms of alcoholic fatty liver - still reversible.
Detailed Answer:
The right upper quadrant pain may be from some inflammation from the alcohol, with fatty liver. But I really don't think you are done-for. Fatty liver, if provoked with more alcohol, can cause some right upper quadrant symptoms.
But you don't have a fever or other symptoms of alcoholic hepatitis (you didn't mention any yellowing of eyes or skin). I think you are likely going to feel better soon. The bigger issue will be staying off alcohol long term.
Here is a site that you might find helpful, that explains a bit about the various stages of alcoholic liver involvement.
